目的:探讨使用二维常规灰阶超声(two-dimensional ultrasonic imaging,2D-US)和自动乳腺全容积成像系统(automated breast volume scanner,ABVS)两种检查方法在诊断乳腺癌边缘脂肪组织声像图中的价值。方法选取2012年7月~2013年3月浙江省人民医院超声科和浙江省舟山医院超声科经超声诊断并行手术治疗的106名患者共143例病灶为研究对象,术前分别使用2D-US和ABVS对病灶进行检查,以病灶边缘脂肪组织出现回声增高、浑浊模糊、内部纤维组织增多,或呈“毛玻璃样”改变作为恶性病变的诊断指标,分析两种检查方法对乳腺癌周围恶性脂肪组织声像图的诊断情况;进一步将肿块按直径大小分为四组,比较两种检查方法对不同直径大小的乳腺癌病灶周围脂肪组织声像图的诊断情况。结果143个病灶分别使用2D-US及ABVS检查,ABVS对脂肪组织声像图诊断的敏感度及准确度高于2D-US,漏诊率低于2D-US(ABVS:68.42%、86.01%、31.58%,2D-US:59.65%、82.52%、40.35%),ABVS对直径超过30 mm的肿块周围恶性脂肪组织声像图的检出率高于2D-US,但两种检查方法对病灶周围脂肪组织声像图诊断的特异度、误诊率差异无统计学意义(P<0.05)。结论 ABVS和2D-US对病变周围的脂肪组织均能较好地进行鉴别,有助于乳腺癌的诊断。

Objective To investigate the diagnostic value of two-dimensional ultrasonic imaging (2D-US) and automat-ed breast volume scanner (ABVS) in sonographic changes of adipose tissue in breast cancer. Methods 106 patients with a total of 143 lesions in Department of Ultrasound, Zhejiang Provincial People's Hospital and Department of Ul-trasound, Zhejiang Provincial Zhoushan Hospital were checked by using 2D-US and ABVS before operation in July 2012 to March 2013. When adipose tissue changed with increased echogenicity, increased fibrosis, the sonographic im-ages were turbidity and blurred or presence of ground-glass-like opacity, these were taken as the diagnostic indicator of malignant lesions. The two methods in diagnosis of adipose tissue which around the breast lesions were compared. According to the diameter size, the masses were divided into 4 groups, and the two methods in diagnosis of adipose tis-sue in different size of lesions were evaluated. Results 2D-US and ABVS were respectively used to check 143 lesions, the sensitivity and accuracy of ultrasonogram diagnosis of adipose tissues by ABVS was higher than by 2D-US, and the rate of misdiagnosis by ABVS was lower than by 2D-US (ABVS:68.42%, 86.01%, 31.58%, 2D-US:59.65%, 82.52%, 40.35%);the detection rate of malignant sonographic changes of adipose tissue by using ABVS was higher than by 2D-US for more than 30 mm diameter of lesions. However, there was no statistical significance differences for specific de-gree and misdiagnosis rate between two methods in diagnosis of sonographic changes of adipose tissue (P < 0.05). Conclusion Both ABVS and 2D-US can make good differential diagnosis of adipose tissues around the breast lesions. The two methods are helpful to the diagnosis of breast cancer.
