目的：观察和评价选择性痔上黏膜切除钉合术（TST）治疗痔病的临床疗效。方法采用随机、对照的研究方法，将2012年6月至2013年9月间在我院肛肠科住院的166例Ⅲ~Ⅳ度内痔及呈梅花状脱出的混合痔患者随机分为TST组和PPH组，分别采用TST术和PPH术，比较观察两组患者的治愈率、术后尿潴留、肛门坠胀、疼痛感、吻合口狭窄等情况。结果两组临床疗效比较，差异无统计学意义。TST组在术后尿潴留、肛门坠胀疼痛感、吻合口狭窄等方面明显优于PPH组（P＜0.05）。结论 TST对Ⅲ~Ⅳ度内痔及梅花状脱出的混合痔患者的治疗效果与PPH相似，但TST组对肛门功能损伤小，术后尿潴留发生率低，肛门坠胀疼痛感轻，持续时间短，无吻合口狭窄发生。
목적：관찰화평개선택성치상점막절제정합술（TST）치료치병적림상료효。방법채용수궤、대조적연구방법，장2012년6월지2013년9월간재아원항장과주원적166례Ⅲ~Ⅳ도내치급정매화상탈출적혼합치환자수궤분위TST조화PPH조，분별채용TST술화PPH술，비교관찰량조환자적치유솔、술후뇨저류、항문추창、동통감、문합구협착등정황。결과량조림상료효비교，차이무통계학의의。TST조재술후뇨저류、항문추창동통감、문합구협착등방면명현우우PPH조（P＜0.05）。결론 TST대Ⅲ~Ⅳ도내치급매화상탈출적혼합치환자적치료효과여PPH상사，단TST조대항문공능손상소，술후뇨저류발생솔저，항문추창동통감경，지속시간단，무문합구협착발생。
Objective To observe and evaluate the selective hemorrhoid studs of mucosal resection combined surgery (TST) the clinical curative effect of treating hemorrhoids sickness. Methods A randomized, controlled study method, will be on June 6, 2012 to September 2013 in our hospital 166 cases of anorectal hospital III~Ⅳdegrees of hemorrhoids and plum blossom shaped out mixed hemorrhoid patients were randomly divided into TST and PPH group, respectively, using TST and PPH, the cure rate of the two groups of patients were studied, postoperative urinary retention, anus belly, pain, anastomotic stricture, and so on and so forth. Results Two groups of clinical curative effect comparison, there was no statistically significant difference. TST in postoperative urinary retention, anal the belly pain, anastomotic stricture, etc obviously better than that of PPH group (P<0.05). Conclusion TST of hemorrhoids and III~Ⅳdegrees plum blossom shape out of the treatment effect of patients with mixed hemorrhoids PPH similar, but the TST group of anal function damage is small, low incidence of postoperative urinary retention, anal belly pain is light, short duration, no anastomotic stenosis occurred.
