目的：观察阴式手术治疗剖宫产后子宫切口憩室(PCSD)的临床疗效。方法：90例剖宫产术后PCSD患者分为3组，每组30例。A组采用普通开腹手术治疗，B组采用宫腔镜手术治疗，C组采用阴式手术治疗，比较各组术中出血量、手术时间、肛门排气时间、术后住院天数、住院治疗费用等指标，随访观察各组术后并发症发生情况。结果：C组手术时间、术中出血量、术后住院时间及治疗费用均低于其他两组(P＜0.05)，肛门排气时间明显早于其他两组(P＜0.05)，术后阴道出血时间晚于其他两组(P＜0.05)；所有患者术后均随访6个月，均未出现严重并发症，各组并发症发生率比较差异无统计学意义(P＞0.05)。结论：阴式手术治疗剖宫产后 PCSD 疗效较为理想，具有多种治疗优点，尤其适合应用于基层医院。
목적：관찰음식수술치료부궁산후자궁절구게실(PCSD)적림상료효。방법：90례부궁산술후PCSD환자분위3조，매조30례。A조채용보통개복수술치료，B조채용궁강경수술치료，C조채용음식수술치료，비교각조술중출혈량、수술시간、항문배기시간、술후주원천수、주원치료비용등지표，수방관찰각조술후병발증발생정황。결과：C조수술시간、술중출혈량、술후주원시간급치료비용균저우기타량조(P＜0.05)，항문배기시간명현조우기타량조(P＜0.05)，술후음도출혈시간만우기타량조(P＜0.05)；소유환자술후균수방6개월，균미출현엄중병발증，각조병발증발생솔비교차이무통계학의의(P＞0.05)。결론：음식수술치료부궁산후 PCSD 료효교위이상，구유다충치료우점，우기괄합응용우기층의원。
Objective:To observe the clinical effect of vaginal operation in the treatment of previous cesarean scar defect(PCSD). Methods:90 cases of cesarean section patients with PCSD were divided into 3 groups with 30 cases in each.A group were given ordinary laparotomy operation treatment.B group were given hysteroscopy operation treatment.C group were given transvaginal operation treatment.The intraoperatve blood soss,operation time,anus exhaust time,postoperative hospitalization day, hospitalization expense and other indexes of each group were compared.The occurrence of postoperative complications of each group were follow-up observated.Results:The operation time,intraoperatve blood soss,postoperative hospitalization day, hospitalization expense of C group were lower than those of other two groups(P<0.05).The anus exhaust time was significantly earlier than that of other two groups(P<0.05).The postoperative vaginal bleeding time was later than that of other two groups(P<0.05).All cases were followed up for 6 months.There were no serious complications.There were no significant difference between the incidence rates of complications in each group(P>0.05).Conclusion:Transvaginal operation in the treatment of cesarean section PCSD has ideal curative effect,and has a variety of therapeutic advantages.It is especially suitable for application in basic hospital.
