目的为杜绝用药差错事件的发生，提高患者用药安全，对用药接近失误进行监控。方法每天由药师填写报告单，每周由班组长汇总，每月上报职能部门并进行分析总结。结果门诊药房开展PDCA后，每天接近失误发生件数由原来的23件下降到9件；通过防范措施，2012年接近失误数据接近目标值≤3‰。结论采取适当的防范措施降低了用药接近失误的发生率，保证了患者的安全。
목적위두절용약차착사건적발생，제고환자용약안전，대용약접근실오진행감공。방법매천유약사전사보고단，매주유반조장회총，매월상보직능부문병진행분석총결。결과문진약방개전PDCA후，매천접근실오발생건수유원래적23건하강도9건；통과방범조시，2012년접근실오수거접근목표치≤3‰。결론채취괄당적방범조시강저료용약접근실오적발생솔，보증료환자적안전。
Objective To prevent the happening of medication errors, improve the medication safety of patients and supervise the medication near misses. Methods The report forms are fil ed by the pharmacists each day and col ected by the team leader each week. The result should be reported to the related department and analyzed and summarized each month. Results After the PDCA cycle, the number of near misses was decreased from 23 to 9 each day. The gap between the data of near misses and the target was ≤3‰ through the preventive measures in 2012. Conclusion The use of proper preventive measures can reduce the happening rate of near misses and guarantee the patients' safety.
