目的：探讨预激综合症合并三度房室传导阻滞的的特征及诊治。方法选择我院预激综合症合并三度房室传导阻滞的患者9例，对其心电图特点及诊治进行回顾分析。结果本组9例患者，合并器质性心脏病2例均给予相应治疗使三度房室传导阻滞消失。2例合并心房颤动者，1例自行终止，另一例行电复律治疗。7例未查到有器质性心脏病者，均植入永久起搏器。结论预激综合症并三度房室传导阻滞，针对病因给予相应治疗能恢复者可行动态观察。对于持续三度房室传导阻滞伴黑曚或晕厥者，可省去电生理检查及射频消融治疗，而直接植入永久起搏器。
목적：탐토예격종합증합병삼도방실전도조체적적특정급진치。방법선택아원예격종합증합병삼도방실전도조체적환자9례，대기심전도특점급진치진행회고분석。결과본조9례환자，합병기질성심장병2례균급여상응치료사삼도방실전도조체소실。2례합병심방전동자，1례자행종지，령일례행전복률치료。7례미사도유기질성심장병자，균식입영구기박기。결론예격종합증병삼도방실전도조체，침대병인급여상응치료능회복자가행동태관찰。대우지속삼도방실전도조체반흑몽혹훈궐자，가성거전생리검사급사빈소융치료，이직접식입영구기박기。
ObjectiveTo study the ECG feature, diagnosis and treatment of pre-excitation syndrome with third degree atrioventricular block.Methods 9 cases with pre-excitation syndrome with the third degree atrioventricular block were retrospectively analyzed for their ECG feature, diagnosis and treatment.Results Among total 9 cases, having suffered from organic heart disease, the ECG morphologies of third degree atrioventricular block disappeared on positive medication in 2 cases. In other 2 cases of pre-excitation syndrome with atria fi brillation, the termination of atrial fi brillation arrythmia spontaneously occurred in one case; the direct electrical cardioversion for atrial fi brillation was given in another case. The permanent cardiac pacemakers were implanted in the other 7cases diagnosed as non organic heart diseases.Conclusion If the arrhythmia of third degree atrioventricular block was able to recover to abnormality in only connection with etiological treatment, we observed the arrhythmia for these cases with pre-excitation syndrome with the third degree atrioventricular block. Moreover, we implanted the permanent cardiac pacemakers, omitting directly electrophysiological examination and radio frequency ablation, for the cases having the third degree atrioventricular block with syncope and blackout attacks.
