目的:对胸腔积液患者的胸水脱落细胞进行DNA倍体分析和癌胚抗原检测,将两种方法进行比较,评估其诊断价值。方法选取经过临床确诊的56例胸腔积液患者作为观察组,56例良性胸腔积液患者作为对照组,对胸水脱落细胞进行DNA倍体分析和癌胚抗原检测。结果 DNA倍体分析的特异度为80.36%,灵敏度为83.93%,Youden指数为0.7029;CEA检测的特异度为96.43%,灵敏度为60.71%,Youden指数为0.5714,两种诊断方法在灵敏度和特异度方面差异有统计学意义(P<0.05,P<0.01),而在Youden指数方面差异无统计学意义(P>0.05)。结论 DNA倍体分析检测胸腔积液的灵敏度和特异度更高,如果和CEA联用,诊断价值更高。

Objective To assess the value of DNA ploidy analysis and carcinoembryonic antigen in the diagnosis of pleural effu-sion by comparing the two methods applied in the detection of hydrothorax exfoliative cells of the patients with pleural effusion. Methods 56 patients diagnosed with pleural effusion clinically were selected as the observation group, and other 56 patients with benign pleural effusion were selected as the control group. The hydrothorax exfoliative cells of the patients were detected by DNA ploidy analysis and CEA. Results The specificity of the DNA ploidy analysis was 80.36%, the sensitivity was 83.93%, Youden in-dex was 0.7029; The specificity of the CEA detection was 96.43%, the sensitivity was 60.71%, the Youden's index was 0.5714. There was significant difference in sensitivity between the two detection methods (P<0.05) as well as in specificity (P<0.01), but not in the Youden's index (P>0.05). Conclusion The sensitivity and specificity of DNA ploidy analysis are higher, and its diagno-sis value will be higher if it is combined with CEA detection.
