目的：对肝左右叶联合切除治疗复杂原发性肝胆管结石的疗效进行探讨与分析。方法：2010年12月-2013年12月收治复杂原发性肝胆管结石患者86例，对其临床资料进行回顾性分析，依照入院前后时间随机给予其中43例肝左右叶联合切除治疗(甲组)，另43例采用肝叶与肝段切除治疗(乙组)，对两组的治疗效果进行观察与对比。结果：通过相应治疗，在治疗优良率上，甲组95.3%(41/43)，乙组76.7%(33/43)，两组差异具有统计学意义(P＜0.05)；在并发症发生率上，甲组4.65%(2/43)，乙组30.2%，甲组显著少于乙组，差异具有统计学意义(P＜0.05)。结论：对于复杂原发性肝胆管结石患者来说，运用肝左右叶联合切除治疗可起到显著的疗效，能够降低并发症发生率和减少残留结石，使患者的生活质量得到提高，值得推广。

목적：대간좌우협연합절제치료복잡원발성간담관결석적료효진행탐토여분석。방법：2010년12월-2013년12월수치복잡원발성간담관결석환자86례，대기림상자료진행회고성분석，의조입원전후시간수궤급여기중43례간좌우협연합절제치료(갑조)，령43례채용간협여간단절제치료(을조)，대량조적치료효과진행관찰여대비。결과：통과상응치료，재치료우량솔상，갑조95.3%(41/43)，을조76.7%(33/43)，량조차이구유통계학의의(P＜0.05)；재병발증발생솔상，갑조4.65%(2/43)，을조30.2%，갑조현저소우을조，차이구유통계학의의(P＜0.05)。결론：대우복잡원발성간담관결석환자래설，운용간좌우협연합절제치료가기도현저적료효，능구강저병발증발생솔화감소잔류결석，사환자적생활질량득도제고，치득추엄。
Objective:To evaluate the effect of left lobe of liver resection in the treatment of complicated primary hepatic bile duct stones.Methods:86 cases with complicated primary hepatic bile duct stones were selected from December 2010 to December 2013. We retrospectively analyzed the clinical data of them.In accordance with the admission time, 43 cases were given the left lobe of liver resection(group A),and the other 43 cases with liver lobes and hepatic segment resection(group B).We observed and compared the therapeutic effect of two groups.Results:Through the corresponding treatment,in the treatment of the excellent and good rate,95.3% in group A(41/43),76.7% (33/43) in group B.The difference was statistically significant(P<0.05).In the complication rate,the group A was 4.65%(2/43),and the group B was 30.2%.The difference was statistically significant(P<0.05). Conclusion:For the complicated primary hepatic bile duct stones,the left lobe of liver resection in treatment can play a significant effect.It can reduce the incidence of complications and reduce the residual stone,and improve the quality of life of patients,so it is worthy of promotion.
