目的：探讨上海市嘉定区安亭镇家庭医生制模式下老年人中医健康管理方法的可行性和有效性。方法：2013年对安亭镇黄渡社区卫生服务中心的9个站点开展老年人中医健康管理方法，分析比较实施前后的效果。结果：确立家庭医生制模式下中医全科团队各成员在老年中医健康管理中的角色与程序，建立绩效考核办法，实施1年后老年人中医体质知晓率、中医保健知识知晓率和中医药保健依从率均较实施前明显提高(P＜0.01)，对社区服务满意度也由实施前的71.3%提高到95.0%( P＜0.01)。结论：以家庭医生制为核心、中医全科团队参与的老年人中医健康管理方法具有可操作性、实用性和有效性。

Objective:To explore the feasibility and effectiveness of the elderly traditional Chinese medicine health management method under family doctor system mode in Anting Town,Jiading District,Shanghai City.Methods:The elderly traditional Chinese medicine health management method was carried out at 9 sites in Huangdu Community Service Center of Anting Town in 2013. The effects before and after the implementation were analyzed and compared.Results:Under family doctor system mode,in the elderly traditional Chinese medicine health management,the role and procedure of each member of traditional Chinese medicine general team were established.The performance appraisal method was established.After 1 year of the implementation,in the elderly patients,the awareness rate of old Chinese medicine physique,the awareness rate of Chinese medicine health,the compliance rate of traditional Chinese medicine health care were obviously improved than those before the implementation(P<0.01).The satisfaction of community service before the implementation was from 71.3% to 95%(P<0.01).Conclusion:The elderly traditional Chinese medicine health management method has the family doctor system as the core,and has the participation of traditional Chinese medicine general team.It has operability and practicability and effectiveness.
