目的：比较宫腔镜手术与刮宫术治疗子宫内膜息肉(EP)型不孕症患者激素表达水平及预后的影响。方法选取本院2010年1月~2013年3月不孕门诊诊治EP型不孕症患者85例为观察组，随机分为宫腔镜亚组45例、刮宫亚组40例，分别予宫腔镜手术与刮宫术治疗，并对患者进行1年的随访；另选择同期因男性因素或输卵管因素不孕就诊、并经阴道B型超声及宫腔镜检查除外EP和其他内膜病变的已婚妇女43例为对照组。观察和比较3组的激素表达水平及预后情况。结果3组的雌二醇(E2)、雌激素受体(ER)、孕激素受体(PR)表达水平比较差异无统计学意义(P>0.05)；宫腔镜亚组的1年内妊娠率(77.78%)明显高于刮宫亚组(40.00%)(P<0.05)；宫腔镜亚组的EP复发率(6.67%)低于刮宫亚组(15.00%)(P<0.05)。结论宫腔镜手术治疗EP相对于刮宫术，其日后受孕率高，复发率低。
목적：비교궁강경수술여괄궁술치료자궁내막식육(EP)형불잉증환자격소표체수평급예후적영향。방법선취본원2010년1월~2013년3월불잉문진진치EP형불잉증환자85례위관찰조，수궤분위궁강경아조45례、괄궁아조40례，분별여궁강경수술여괄궁술치료，병대환자진행1년적수방；령선택동기인남성인소혹수란관인소불잉취진、병경음도B형초성급궁강경검사제외EP화기타내막병변적이혼부녀43례위대조조。관찰화비교3조적격소표체수평급예후정황。결과3조적자이순(E2)、자격소수체(ER)、잉격소수체(PR)표체수평비교차이무통계학의의(P>0.05)；궁강경아조적1년내임신솔(77.78%)명현고우괄궁아조(40.00%)(P<0.05)；궁강경아조적EP복발솔(6.67%)저우괄궁아조(15.00%)(P<0.05)。결론궁강경수술치료EP상대우괄궁술，기일후수잉솔고，복발솔저。
Objective To compare the influence of hysteroscopic operation and curettage on hormone expression level and prognosis in patients with infertility and endometrial polyp (EP). Methods 85 cases of patients with infertility and endometrial polyp treated in infertility out-patient department of the hospital from January 2010 to March 2013 were se-lected as observation group,then randomly divided into hysteroscopy subgroup(45 cases) and curettage subgroup(40 cases), they were treated with hysteroscopic operation and curettage respectively,and 1-year follow-up was carried out.At the same period,43 married women who were infertility due to male factor or fallopian tube factor were selected as control group,they all excluded EP and other endometrial diseases via transvaginal B type ultrasound examination and hystero-scopic examination.Hormone expression level and prognosis was observed and compared respectively among the three groups. Results The expression of estradiol (E2),estrogen receptor (ER),progesterone receptor (PR)in three groups had no significant difference(P>0.05).1-year pregnancy rate in hysteroscopy subgroup(77.78%)was significantly higher than that in curettage subgroup(40.00%,P<0.05).Recurrence rate in hysteroscopy subgroup(6.67%)was lower than that in curettage subgroup (15.00%,P<0.05). Conclusion Compared with curettage operation,hysteroscopy operation has higher future pregnancy rate and lower recurrence rate in the treatment of EP.
