目的:探讨不同程度宫颈病变的 HPV基因型分布特点及高危型 HPV E6/E7 mRNA在单一、多重感染病例中的表达,以及其对 HPV致病力的影响。方法收集不同程度宫颈病变石蜡组织标本共236份。采用基因芯片技术对236份标本进行23种 HPV基因型分型检测,并采用Taqman逆转录-PCR技术测定高危型 HPV E6/E7 mRNA的表达。结果 HPV阳性率为65.3%(154/236),其中包含了233个 HPV亚型(单一或多重)感染,60.9%(142/233)病例可检测出 E6/E7 mRNA的表达,HPV阳性率和 E6/E7 mRNA表达水平随宫颈病变级别的上升而升高(P<0.05)。在大于或等于 CINⅡ的高危亚型感染病例中, HPV45的 E6/E7 mRNA表达率最高(93.3%),其次为 HPV16(81.0%)、HPV18(78.4%)。在大于或等于 CINⅡ的病变中,93.7%(118/126)病例可检测到 E6/E7 mRNA的表达,而小于CINⅡ的病变中仅有39.3%(11/28)的病例可检测到 E6/E7 mR-NA的表达。在所有不同程度宫颈病变中,多重感染病例E6/E7 mRNA表达率较单一感染病例高(61.2%vs.38.8%,P<0.05)。结论在任何程度的宫颈病变中,HPV多重感染,尤其 HPV45的 E6/E7 mRNA高表达水平可能会提高 HPV的致病力。

목적:탐토불동정도궁경병변적 HPV기인형분포특점급고위형 HPV E6/E7 mRNA재단일、다중감염병례중적표체,이급기대 HPV치병력적영향。방법수집불동정도궁경병변석사조직표본공236빈。채용기인심편기술대236빈표본진행23충 HPV기인형분형검측,병채용Taqman역전록-PCR기술측정고위형 HPV E6/E7 mRNA적표체。결과 HPV양성솔위65.3%(154/236),기중포함료233개 HPV아형(단일혹다중)감염,60.9%(142/233)병례가검측출 E6/E7 mRNA적표체,HPV양성솔화 E6/E7 mRNA표체수평수궁경병변급별적상승이승고(P<0.05)。재대우혹등우 CINⅡ적고위아형감염병례중, HPV45적 E6/E7 mRNA표체솔최고(93.3%),기차위 HPV16(81.0%)、HPV18(78.4%)。재대우혹등우 CINⅡ적병변중,93.7%(118/126)병례가검측도 E6/E7 mRNA적표체,이소우CINⅡ적병변중부유39.3%(11/28)적병례가검측도 E6/E7 mR-NA적표체。재소유불동정도궁경병변중,다중감염병례E6/E7 mRNA표체솔교단일감염병례고(61.2%vs.38.8%,P<0.05)。결론재임하정도적궁경병변중,HPV다중감염,우기 HPV45적 E6/E7 mRNA고표체수평가능회제고 HPV적치병력。
Objective To investigate the HPV genotype distribution characteristics in the patients with different degrees of cer-vical diseases and the expression of high-risk HPV E6/E7 mRNA in the patients with single or multiple infections and its influence on the HPV pathogenicity.Methods A total of 236 formalin-fixed,paraffin-embedded specimens were collected from women with different degrees of cervical diseases.The gene chip technique was adopted to conduct the genotype detection.The in 23 kinds of HPV genotype on 236 specimens.The TaqmanRT-PCR technique was adopted to measure the expression of high-risk HPV E6/E7 mRNA.Results The HPV positive rate was 65.3%(154/236),including 233 cases of HPV subgenotye infection(single or multi-ple),the expression of HPV E6/E7 mRNA was detected in 60.9% of cases(142/233),the HPV positive rate and the expression level of HPV E6/E7 mRNA were increased with the increase of the cervical lesion grade(P<0.05).In the high-risk infection cases of lesion≥CINⅡ,the expression rate of HPV45 E6/E7 mRNA was highest(93.3%),followed by HPV16(81.0%)and HPV18 (78.4%).In the lesions ≥CINⅡ,the expression of E6/E7 mRNA was detected in 93.7% of cases(118/126),while which in le-sions < CINⅡwas detected in only 39.3 % of cases(11/28).In different degrees of cervical lesions,the expression rate of E6/E7 mRNA in the cases of multiple infection was higher than that in the caes of single infection(61.2%vs.38.8%,P<0.05).Conclu-sion In any degree of cervical lesion,HPV multiple infection,especially high expression level of HPV45 E6/E7 mRNA could in-crease the HPV pathogenicity.
