目的:观察研究高频震荡通气治疗新生儿胎粪吸入性综合征合并持续肺动脉高压的疗效。方法:选取新生儿胎粪吸入性综合征合并持续肺动脉高压40例,随机分为试验组与对照组,各20例。试验组患儿给予高频震荡通气治疗,对照组患儿给予常频机械通气治疗,10 h后,观察比较两组患儿治疗前后氧合指数和肺动脉压力。结果:10h后试验组患儿生存率为85%,高于对照组患儿生存率65%;试验组治疗后氧合指数与肺动脉压力明显优于对照组的氧合指数与肺动脉压力。结论:高频震荡通气可以改善新生儿胎粪吸入性综合征合并持续肺动脉高压的氧合指数,降低肺动脉压力,提高患儿的存活率,具有重要的临床意义,值得广泛推广。

목적:관찰연구고빈진탕통기치료신생인태분흡입성종합정합병지속폐동맥고압적료효。방법:선취신생인태분흡입성종합정합병지속폐동맥고압40례,수궤분위시험조여대조조,각20례。시험조환인급여고빈진탕통기치료,대조조환인급여상빈궤계통기치료,10 h후,관찰비교량조환인치료전후양합지수화폐동맥압력。결과:10h후시험조환인생존솔위85%,고우대조조환인생존솔65%;시험조치료후양합지수여폐동맥압력명현우우대조조적양합지수여폐동맥압력。결론:고빈진탕통기가이개선신생인태분흡입성종합정합병지속폐동맥고압적양합지수,강저폐동맥압력,제고환인적존활솔,구유중요적림상의의,치득엄범추엄。
Objective To study the high-frequency oscillatory ventilation in treatment of neonatal meconium aspiration syndrome with persistent pulmonary hypertension. Method 40 cases neonatal meconium aspiration syndrome with persistent pulmonary hypertension , were randomly divided into experimental group and control group,20 cases in each. The test group were given high-frequency oscillatory ventilation in the treatment,the control group were given conventional mechanical ventilationtreatment,after 10 hours,were observed before and after treatment in the two groups were compared the oxygenation index and pulmonary artery pressure. Results The survival rate of the test group was85%,of 85% of the cantrol group was 65% The survival rate of the test groups of children with survival test results was higher than that of the control group after 10h,the survival rate ;the test group after treatment of oxygenation index and pulmonary artery pressure was significantly better than the control group of oxygenation index and pulmonary artery pressure. Conclusion High frequency os-cillation ventilation can improveneonatal meconium aspiration of persistent pulmonary hypertension withoxygenation index syndrome,reduce pulmonary artery pressure,improve the survival rate of children,and has important clinical significance,is worth popularizing widely.
