目的：评价两种手术方法治疗复杂胫骨平台骨折的临床疗效。方法对78例复杂胫骨平台骨折的临床资料进行回顾性分析，依据手术入路方式分为内外侧双切口入路和膝前Y型切口入路两组，比较两种手术方法治疗复杂胫骨平台骨折的疗效。结果膝前Y型切口入路组较内外侧双切口入路组显露好，手术时间相对较短（ P＜0．05），但术中出血量、术后引流量两组比较差异无统计学意义（ P＞0．05）；膝前Y型切口组出现皮肤坏死机率高于双切口组（ P＜0．05），感染机率方面比较差异无统计学意义（ P＞0．05）；在骨折愈合时间及膝关节功能恢复方面，两组比较差异无统计学意义（ P＞0．05）。结论两种手术方法均是治疗复杂胫骨平台骨折的有效方法，应根据适应症范围合理选择。

Objective To investigate the clinical effect of two approaches on complex tibial plateau frac-ture.Methods 78 clinical date of complex tibial plateau fractures were selected and were analyzed with retrospective method.Date were divided into bilateral approach group ,Y type approach group .Clinical effect of two groups were con-trasted.Results The Y type approach group reduced operative time and had better exposure compared with bilateral ap -proach group.However,there is no significant difference in blood loss ,healing time of fracture and the function of evalua-tion in two groups.Bilateral approach group had fewer complications compared with Y type approach group .Conclusions The two groups both showed effective .The operative methods should be selected according to indications .
