目的:探讨健康教育联合强化行为管理在糖尿病周围神经病变中的应用效果。方法2012年1月~2013年1月我科住院的2型糖尿病合并周围神经病变患者70例,按照随机数字表分为对照组和观察组,各35例。对照组进行常规护理及糖尿病健康教育管理。观察组在常规护理、健康教育管理的基础上进行强化行为管理。比较两组1年后自我管理行为、生化指标变化及神经功能评分。结果随访1年后,观察组饮食、控制体重、运动、用药以及监测血糖等自我管理行为优于对照组,有显著性差异(P<0.05)。随访1年后,观察组各项指标水平明显低于对照组,有显著性差异(P<0.05)。随访1年后,两组TCSS评分均有所下降,但观察组下降更明显,与对照组比较有显著性差异(P<0.05)。结论健康教育联合强化行为管理可降低糖尿病周围神经病变患者的血糖、血脂水平,提高患者自我管理能力,改善神经功能,可应用于临床。

목적:탐토건강교육연합강화행위관리재당뇨병주위신경병변중적응용효과。방법2012년1월~2013년1월아과주원적2형당뇨병합병주위신경병변환자70례,안조수궤수자표분위대조조화관찰조,각35례。대조조진행상규호리급당뇨병건강교육관리。관찰조재상규호리、건강교육관리적기출상진행강화행위관리。비교량조1년후자아관리행위、생화지표변화급신경공능평분。결과수방1년후,관찰조음식、공제체중、운동、용약이급감측혈당등자아관리행위우우대조조,유현저성차이(P<0.05)。수방1년후,관찰조각항지표수평명현저우대조조,유현저성차이(P<0.05)。수방1년후,량조TCSS평분균유소하강,단관찰조하강경명현,여대조조비교유현저성차이(P<0.05)。결론건강교육연합강화행위관리가강저당뇨병주위신경병변환자적혈당、혈지수평,제고환자자아관리능력,개선신경공능,가응용우림상。
Objective To study the value of health education combinated with intensive behavioral management in the di-abetic peripheral neuropathy .Methods 70 patients with type 2 diabetes mellitus with peripheral neuropathy from January 2012 to January 2013 in our hospital were divided in to two groups .Patients in the control group were received routine nurs-ing and health education management and patients in the observation group were received intensive behavior management based on routine nursing and health education management .After one year ,self-management behaviors ,changes in bio-chemical markers and neurological function were compared in the two groups .Results Follow-up of 1 year ,diet ,self-man-agement behaviors such as weight control ,exercise ,medication and blood glucose monitoring was better in the observation group than that in the control group ,there was statistically significant (P<0 .05) .After 1 year of treatment ,the indicators in the observation group were significantly lower than that in the control group ,there were statistically significant (P<0 . 05) .After 1 year of follow-up ,TCSS score in both groups declined ,but TCSS score in the observation group decreased more significantly ,there was statistically significant between two groups (P<0 .05) .Conclusions Health education combinated with intensive behavioral management in the diabetic peripheral neuropathy can reduce blood glucose ,and blood lipids ,im-prove patient self-management skills and nerve function ,is worthy of clinical application .
