目的:探讨超声弹性成像对乳腺肿瘤的鉴别诊断价值。方法对98例107个乳腺结节进行超声弹性成像检查,并与病理结果比较。弹性成像按5分法分级。结果107个乳腺结节,组织病理检查结果为良性结节68个,恶性结节39个。超声弹性成像评分≤3分(良性)67个结节,>3分(恶性)40个结节。良性结节超声弹性成像与病理诊断符合率为86.8%(59/68);恶性结节超声弹性成像与病理诊断符合率为79.5%(31/39)。以弹性成像评分>3分,并且弹性成像病灶面积与二维超声面积比≥1.5为恶性肿瘤诊断标准,诊断乳腺恶性病变的敏感性、特异性、准确性分别为79.4%、97.1%、和90.7%。结论弹性成像可有效评估组织硬度,定量参数面积比有助于乳腺肿块良恶性的鉴别。

목적:탐토초성탄성성상대유선종류적감별진단개치。방법대98례107개유선결절진행초성탄성성상검사,병여병리결과비교。탄성성상안5분법분급。결과107개유선결절,조직병리검사결과위량성결절68개,악성결절39개。초성탄성성상평분≤3분(량성)67개결절,>3분(악성)40개결절。량성결절초성탄성성상여병리진단부합솔위86.8%(59/68);악성결절초성탄성성상여병리진단부합솔위79.5%(31/39)。이탄성성상평분>3분,병차탄성성상병조면적여이유초성면적비≥1.5위악성종류진단표준,진단유선악성병변적민감성、특이성、준학성분별위79.4%、97.1%、화90.7%。결론탄성성상가유효평고조직경도,정량삼수면적비유조우유선종괴량악성적감별。
Objective To evaluate the value of elastosonography in the differential diagnosis of breast tumor . Methods Ninety-eight patients with 107 breast lesions were examined by elastosonography ,and the result was compare with pathological result .The grading of elastosonography was done according to 5-score evaluating method .Results The histopathological examination showed that 61 benign lesions and 39 malignant lesions in 107 breast lesions .There were 67 lesions(benign) with elastosonography score ≤3 and 40 lesions(malignant) >3.The coincidence rate of elastosonography and histopathological examination in the diagnosis of benign lesions was 86.8%(59/68),which was 79.5%(31/39) in the diagnosis of malignant lesions .Based on the diagnostic standard for malignant lesions which was the elastosonography score >3 and the ratio of lesion traced area in strain images versus in B-mode images≥1 .5 ,the sensitivity ,specificity and accuracy of elastosonograpy in the diagnosis of malignant breast lesions were 79.4%,97.1%and 90.7%,respectively. Conclusion Elastosonography can evaluate the tissue hardness effectively ,the area ratio of quantitative parameters might benefit the differential diagnosis of benign and malignant lesions .
