目的:探研大剂量氨溴索治疗重症肺炎的临床疗效。方法本院在2008年6月~2013年7月收治56例重症肺炎患者,随机分为观察组30例和对照组26例,两组均给予常规治疗,观察组再给予静脉滴注大剂量氨溴索900 mg,1次/d;对照组再给予静脉滴注氨溴索60~180 mg/d,2次/d。两组疗程均为8~14 d。结果观察组治疗后发热、咳嗽症状与肺部湿性啰音消失时间均比对照组缩短( P<0.05);两组治疗后血氧分压、氧合指数均高于治疗前(P<0.05),观察组治疗后血氧分压、氧合指数均高于对照组(P<0.05);两组治疗后IL-8、TNF-α均明显低于治疗前(P<0.05),观察组治疗后IL-8、TNF-α均低于对照组(P<0.05)。结论大剂量氨溴索治疗重症肺炎疗效确切,能够更快缩短症状和体征消失时间,更好改善氧合情况。

목적:탐연대제량안추색치료중증폐염적림상료효。방법본원재2008년6월~2013년7월수치56례중증폐염환자,수궤분위관찰조30례화대조조26례,량조균급여상규치료,관찰조재급여정맥적주대제량안추색900 mg,1차/d;대조조재급여정맥적주안추색60~180 mg/d,2차/d。량조료정균위8~14 d。결과관찰조치료후발열、해수증상여폐부습성라음소실시간균비대조조축단( P<0.05);량조치료후혈양분압、양합지수균고우치료전(P<0.05),관찰조치료후혈양분압、양합지수균고우대조조(P<0.05);량조치료후IL-8、TNF-α균명현저우치료전(P<0.05),관찰조치료후IL-8、TNF-α균저우대조조(P<0.05)。결론대제량안추색치료중증폐염료효학절,능구경쾌축단증상화체정소실시간,경호개선양합정황。
Objective To investigate the clinical effect of large doses of ambroxol in the treatment of severe pneumonia. Methods 56 severe pneumonia patients were randomly divided into the observation group (30 cases) and the control group (26 cases). Both groups were given conventional treatment. The observation group was given large doses of intravenous ambroxol with 900 mg, 1 time/d, while the control group was given intravenously ambroxol with 60~180 mg/d, 2 times/d. The treatment lasted 8~14 days. Results After the treatment, the improvement of fever, cough and lungs moist rales disappearing time was more pronounced in the observation group than in the control group ( P<0. 05 ) . The indexes of blood oxygen pressure and oxygenation were obviously higher in the observation group than in the control group (P<0. 05). The levels of IL-8 and TNF-αwere lower after the treatment than before (P<0. 05), and the decrease was more pronounced in the observation group than in the control group (P<0. 05). Conclusion The effect of large dose of ambroxol hydrochloride is better in the treatment of severe pneumonia, which can quickly reduce symptoms and signs disappeared time and improve oxygenation.
