目的:观察运动(呼吸和步行)及渐进性肌肉放松训练对缓解期慢性阻塞性肺疾病( COPD)患者运动耐力、日常生活能力及情绪的影响。方法55例缓解期中、重度COPD患者随机分为对照组31例、训练组24例。对照组给予药物治疗,康复训练组除予以药物治疗外,配合进行运动和渐进性肌肉放松训练,持续观察6个月。观察期前后分别评价运动耐力(6MWD)、呼吸困难程度(Borg评分)和日常生活能力(ADL评分),同时进行焦虑抑郁程度和肺功能评定。结果 COPD患者训练后6MWD明显高于训练前和对照组(均P<0.05), ADL评分明显增加(P<0.05),而Borg评分明显减轻(P<0.05),焦虑、抑郁评定亦显著降低(P<0.05),但训练前后患者肺功能无明显差异(P>0.05)。结论运动及渐进性肌肉放松训练尽管对缓解期COPD患者肺功能无显著影响,但可增加其运动耐力和ADL能力,减轻呼吸困难和焦虑抑郁程度。

목적:관찰운동(호흡화보행)급점진성기육방송훈련대완해기만성조새성폐질병( COPD)환자운동내력、일상생활능력급정서적영향。방법55례완해기중、중도COPD환자수궤분위대조조31례、훈련조24례。대조조급여약물치료,강복훈련조제여이약물치료외,배합진행운동화점진성기육방송훈련,지속관찰6개월。관찰기전후분별평개운동내력(6MWD)、호흡곤난정도(Borg평분)화일상생활능력(ADL평분),동시진행초필억욱정도화폐공능평정。결과 COPD환자훈련후6MWD명현고우훈련전화대조조(균P<0.05), ADL평분명현증가(P<0.05),이Borg평분명현감경(P<0.05),초필、억욱평정역현저강저(P<0.05),단훈련전후환자폐공능무명현차이(P>0.05)。결론운동급점진성기육방송훈련진관대완해기COPD환자폐공능무현저영향,단가증가기운동내력화ADL능력,감경호흡곤난화초필억욱정도。
Objective To investigate the effect of exercise and progressive muscle relaxation training on ex-ercise endurance, activities of daily living ( ADL) , depressive and anxious symptoms patients with stable chronic ob-structive pulmonary disease ( COPD ) . Methods 55 patients with stable COPD were randomly divided into two groups. The training group consisted of 24 cases and the control group consisted of 31 cases. Both groups received medicine therapy. Meanwhile, the training group received breathing training, walking training and progressive muscle relaxation training for 6 months. The endurance of exercise (6MWD), dyspnea ratings (Borg scale), ADL, depres-sive and anxious symptoms assessment and lung function were evaluated before and after the training program. Re-sults Although lung function did not change significantly after training, exercise endurance, dyspnea, ADL and de-pressive and anxious symptoms improved significantly in the training group. Conclusion Exercise and progressive muscle relaxation training can markedly improve the exercise endurance and ADL, as well as relieve the dyspnea, de-pressive and anxious symptoms in patients with stable COPD.
