目的：比较交锁髓内钉与锁定钢板内固定治疗股骨干骨折的临床疗效，为临床治疗选取内固定术式提供理论依据。方法采用回顾性分析方法，该科于2012年1月-2013年6月收治的40例股骨干骨折，均获得随访，其中20例接受交锁髓内钉内固定术，20例行锁定加压钢板内固定术，对两组患者的平均手术时间、术中出血量、术后48 h引流量、术后48 h血红蛋白值、骨折愈合时间、骨折愈合时膝关节优良率等方面进行比较。结果40例患者均获得随访，随访时间6～18个月。两组在手术时间、膝关节优良率及术后48 h平均血红蛋白值上差异无统计学意义（P＞0．05），在术中出血量、术后48 h切口引流量、骨折愈合时间上差异有统计学意义（P＜0．05）。结论总体来说，交锁髓内钉治疗股骨干骨折疗效优于锁钉钢板，治疗成人股骨干骨折，推荐首选交锁髓内钉。
목적：비교교쇄수내정여쇄정강판내고정치료고골간골절적림상료효，위림상치료선취내고정술식제공이론의거。방법채용회고성분석방법，해과우2012년1월-2013년6월수치적40례고골간골절，균획득수방，기중20례접수교쇄수내정내고정술，20례행쇄정가압강판내고정술，대량조환자적평균수술시간、술중출혈량、술후48 h인류량、술후48 h혈홍단백치、골절유합시간、골절유합시슬관절우량솔등방면진행비교。결과40례환자균획득수방，수방시간6～18개월。량조재수술시간、슬관절우량솔급술후48 h평균혈홍단백치상차이무통계학의의（P＞0．05），재술중출혈량、술후48 h절구인류량、골절유합시간상차이유통계학의의（P＜0．05）。결론총체래설，교쇄수내정치료고골간골절료효우우쇄정강판，치료성인고골간골절，추천수선교쇄수내정。
Objective To compare the clinical curative effects between interlocking intramedullary nails and locking plate internal fixa-tion in the treatment of femoral fractures,and to provide theoretical basis for clinical selection of internal fixation operation.Methods Using retrospective analysis method,we compared the average operation time,intraoperative blood loss,postoperative flow and hemoglo-bin value 48 h after operation,fracture healing time,acceptance rate of fracture healing of the knee joint in 40 cases of femoral fractures admitted to hospital between January 2012 and June 2013 and were followed up,including 20 cases treated with interlocking intramedul-lary nail fixation and 20 routine locking compression plate fixation.Results All the 40 patients were followed up for 6 ~18 months. There were no statistically significant differences between the two groups in operative time,acceptance rate of knee joint function and hemoglobin values 48 hours after operation(P>0.05),while there were statistically significant differences in the intraoperative blood loss,postoperative 48-hour incision volume and fracture healing time(P<0.05).Conclusions In general,the curative effect of inter-locking intramedullary nail for femoral fracture is superior to that of locking compression plate,which is a preferred method for recom-mendation.
