目的：探讨联合胸水癌胚抗原（CEA）、腺苷脱氨酶（ADA）检测在结核性渗出性胸膜炎及恶性胸腔积液中的意义。方法回顾性分析2008年1月-2013年1月在该院住院期间，诊断为结核性渗出性胸膜炎、恶性胸腔积液的148例患者，检测胸腔积液中的CEA及ADA含量，并加以ROC分析。结果在恶性胸腔积液的诊断方向上，ADA和CEA两种指标/方法，灵敏度分别为0．857和0．976，特异度均为1．000，诊断阈值分别为18 U·L-1和2．8μg·L-1，两种方法经配对检验无显著性差异。结论联合检测胸水CEA及ADA对于鉴别结核性渗出性胸膜炎与恶性胸腔积液有着至关重要的意义。

Objective To discuss the value of combined detection of pleural effusion CEA and ADA in tuberculous exudative pleurisy and malignant pleural effusion.Methods A retrospective study was made of 104 patients with tuberculous exudative pleurisy and ma-lignant pleural effusion admitted to No.98 Hospitla of PLA from January ,2008 to January,2013.Pleural effusion CEA and ADA levels were detected and ROC analysis was made.Results From the malignant pleural effusion diagnostic standpoint,the sensitivities of ADA and CEA were 0.857 and 0.976,the specificities were both 1,and the diagnostic thresholds were 18 U·L-1and 2.8 μg·L-1,respec-tively.The two methods had no significant difference by paired test.Conclusions The combined detection of pleural effusion CEA and ADA has crucial importance to tuberculous exudative pleurisy and malignant pleural effusion.
