目的：对比检测胃癌、胃息肉及健康对照者外周血中的中性粒细胞与淋巴细胞比值（ NLR），并分析 NLR在胃癌中潜在的临床意义。方法收集经临床病理诊断的202例胃癌患者、150例非恶性胃息肉患者及186例健康对照者，依据临床血液常规检测得到 NLR，随后在胃癌患者中分析 NLR 与肿瘤的大小、肿瘤 TNM 分期、肿瘤分化等相关性。结果与健康对照组及胃息肉组相比，胃癌患者外周血中 NLR 显著升高（4.12±1.15 vs 2.01±1.28，2.07±0.98；P ﹤0.05）；且 NLR 与肿瘤大小、肿瘤 TNM 临床分期密切相关，在肿瘤大于3 cm、淋巴结转移与远处转移、临床分期晚的患者中 NLR 显著高于其他的胃癌患者（ P ﹤0.05）；而对于不同病理分化程度的患者 NLR 的检测分析发现 NLR 与肿瘤的分化程度无相关性（ P =0.07）。结论胃癌患者的 NLR 显著高于正常人，且与患者的临床 TNM 分期密切相关，因而NLR 可能会成为判断胃癌患者疾病严重程度、治疗效果、预后等的一个新的指标。
목적：대비검측위암、위식육급건강대조자외주혈중적중성립세포여림파세포비치（ NLR），병분석 NLR재위암중잠재적림상의의。방법수집경림상병리진단적202례위암환자、150례비악성위식육환자급186례건강대조자，의거림상혈액상규검측득도 NLR，수후재위암환자중분석 NLR 여종류적대소、종류 TNM 분기、종류분화등상관성。결과여건강대조조급위식육조상비，위암환자외주혈중 NLR 현저승고（4.12±1.15 vs 2.01±1.28，2.07±0.98；P ﹤0.05）；차 NLR 여종류대소、종류 TNM 림상분기밀절상관，재종류대우3 cm、림파결전이여원처전이、림상분기만적환자중 NLR 현저고우기타적위암환자（ P ﹤0.05）；이대우불동병리분화정도적환자 NLR 적검측분석발현 NLR 여종류적분화정도무상관성（ P =0.07）。결론위암환자적 NLR 현저고우정상인，차여환자적림상 TNM 분기밀절상관，인이NLR 가능회성위판단위암환자질병엄중정도、치료효과、예후등적일개신적지표。
Objective To detect the levels of neutrophil to lymphocyte ratio(NLR)in peripheral blood of gastric cancer,gastric polyp and normal controls,and to evaluate its potential clinical significance in cases with gastric cancer. Methods The peripheral blood from 202 cases with gastric cancer,150 cases with gastric polyp and 180 normal controls were collected and obtained,and the NLR of each subjects were collect-ed. Then,we analysed the correlation of NLR and TNM clinical stages,tumor differentiation,etc. Results The NLR level in peripheral blood of gastric cancer is significantly higher than that of other two groups(4. 12 ± 1. 15 vs 2. 01 ± 1. 28,2. 07 ± 0. 98,P ﹤ 0. 05). The cases with nodal invasion,metastasis and late clinical stage have relatively higher levels of NLR( P ﹤ 0. 05). No significant difference in NLR in different tumor differentiation could be observed( P = 0. 07). Conclusion There is a positive association of NLR and clinical stage in gastric cancer,supporting that NLR may be a novel indicator for disease severity,treatment effect and prognosis.
