目的:探讨老年男性慢性阻塞性肺疾病(COPD)与骨质疏松症的关系。方法选择80例老年男性 COPD患者为研究对象,30例年龄匹配的健康体检老年男性为对照组。测定所有研究对象的肺功能、动脉血气水平等肺功能指标。采用双能 X 线骨密度测定仪测定110例研究对象腰椎和股骨近端的骨密度(BMD),记录各组研究对象骨质疏松的发生率。结果 COPD 组腰椎、股骨近端的 BMD 均低于正常对照组( P ﹤0.05)。COPD 组腰椎、股骨近端骨质疏松发生率明显高于对照组。COPD 组中,吸烟患者的 BMD 明显低于不吸烟的患者;未使用激素治疗 COPD 患者的 BMD 明显高于使用激素的患者,差异均有统计学意义( P ﹤0.05)。结论老年男性 COPD 患者 BMD 低于同龄对照组,骨质疏松发生率高于对照组。肺功能指标、吸烟和使用糖皮质激素均可能影响 COPD 患者骨质疏松的发生,且随 COPD 分级的增高,骨质疏松的发生率也增高。

목적:탐토노년남성만성조새성폐질병(COPD)여골질소송증적관계。방법선택80례노년남성 COPD환자위연구대상,30례년령필배적건강체검노년남성위대조조。측정소유연구대상적폐공능、동맥혈기수평등폐공능지표。채용쌍능 X 선골밀도측정의측정110례연구대상요추화고골근단적골밀도(BMD),기록각조연구대상골질소송적발생솔。결과 COPD 조요추、고골근단적 BMD 균저우정상대조조( P ﹤0.05)。COPD 조요추、고골근단골질소송발생솔명현고우대조조。COPD 조중,흡연환자적 BMD 명현저우불흡연적환자;미사용격소치료 COPD 환자적 BMD 명현고우사용격소적환자,차이균유통계학의의( P ﹤0.05)。결론노년남성 COPD 환자 BMD 저우동령대조조,골질소송발생솔고우대조조。폐공능지표、흡연화사용당피질격소균가능영향 COPD 환자골질소송적발생,차수 COPD 분급적증고,골질소송적발생솔야증고。
Objective To study the relationship of chronic obstructive pulmonary diseases(COPD)and osteoporosis( OS)in the old male patients. Methods 80 old male COPD cases were included into this study,and 30 age matched physical examination old male were included into this study and considered as control group. The index of lung function such as arterial blood gas was measured. Bone mineral density(BMD) at the bottom of the lumbar spine and femoral was measured by dual - energy X - ray absorptiometry to analyze the incidence of osteoporosis. Re-sults Lumbar spine and femoral BMD at the bottom of the COPD group were lower than normal control group( P ﹤ 0. 05). The lumbar spine and proximal femoral bone loss rate of COPD group is significantly higher than the control group. The BMD of patients in smoking COPD patients was significantly lower than nonsmoking patients. The BMD of none - using hormone therapy COPD patients was obviously higher than that of patients with using hormones. These differences were statistically significant( P ﹤ 0. 05). Conclusion The BMD in elderly male patients is lower than the control group. The incidence of osteoporosis is higher than the control group. Lung function index,smoking and the use of glucocorticoid are likely to affect the incidence of osteoporosis in patients with COPD,and with the increasing of COPD classification,also increased the incidence of osteoporosis.
