目的:探讨维持性血液透析( MHD)患者脂质代谢紊乱与脑血管事件发生之间的关系。方法:回顾性分析56例发生脑血管事件的MHD患者(病例组)和54例同期病情稳定的MHD患者(对照组)的临床资料。结果:发生脑血管事件的MHD患者低密度脂蛋白胆固醇( LDL-c)水平明显高于对照组。对亚组进行单因素分析发现,与对照组相比,脑出血亚组总胆固醇( TC)明显降低;脑梗死亚组TC、LDL-c明显升高;多因素分析发现脑梗死亚组主要表现为LDL-c升高。结论:MHD患者发生脑血管事件的脂质代谢紊乱主要表现为LDL-c升高为主;总胆固醇降低可能是MHD患者并发脑出血的重要危险因素,LDL-c升高可能是MHD患者并发脑梗死的重要危险因素。

목적:탐토유지성혈액투석( MHD)환자지질대사문란여뇌혈관사건발생지간적관계。방법:회고성분석56례발생뇌혈관사건적MHD환자(병례조)화54례동기병정은정적MHD환자(대조조)적림상자료。결과:발생뇌혈관사건적MHD환자저밀도지단백담고순( LDL-c)수평명현고우대조조。대아조진행단인소분석발현,여대조조상비,뇌출혈아조총담고순( TC)명현강저;뇌경사아조TC、LDL-c명현승고;다인소분석발현뇌경사아조주요표현위LDL-c승고。결론:MHD환자발생뇌혈관사건적지질대사문란주요표현위LDL-c승고위주;총담고순강저가능시MHD환자병발뇌출혈적중요위험인소,LDL-c승고가능시MHD환자병발뇌경사적중요위험인소。
Objective:To investigate the relationship of lipid metab-olism disorders and cerebrovascular events in mainte-nance hemodialysis patients( MHD). Nethods:We retrospectively analyzed the clinical data of 56 MHD patients with cerebrovascular events( case group)and 54 MHD patients with stable state( control group). Results:The level of LDL-c of MHD patients with cere-brovascular events are significantly higher than the controlgroup. The results of subgroup from univariate analysis showed that level of total cholesterol( TC)in Cerebral hemorrhage subgroup significantly lower;the level of TC and LDL-c in Cerebral infarcti -on group significantly rise. Multiariable analysis found that level of LDL-c in cerebral infarction subgroup is significantly rise. Conclu-sion:The lipid metabolism diso-rders of MHD patients with cerebrovascular events is mainly the level of LDL-c rise. The reduce of level of TC may be the important risk factor for MHD patients complicated by cerebral hemorrhage. The rise of level of LDL-c may be the important risk factor for MHD patients complicated bycerebral infarction.
