目的:探讨缬沙坦对冠心病患者脂联素和血管内皮功能的影响。方法将40例冠心病患者随机分为2组,常规治疗组20例给予阿司匹林和酒石酸美托洛尔(倍他乐克),缬沙坦组20例给予常规治疗加缬沙坦。治疗前和治疗8周后,采用超声多普勒法测定血流介导的肱动脉舒张反应( FMD),并检测治疗前后患者血浆脂联素水平和一氧化氮( NO)水平。结果与治疗前比较,常规治疗组肱动脉基础内径、FMD、NO、脂联素水平均无显著变化(P >0.05),缬沙坦组除肱动脉基础内径外,其余指标均显著升高( P<0.05, P <0.01)。与常规治疗组相比,缬沙坦组治疗后FMD、脂联素、NO水平均明显增加( P <0.05, P <0.01)。2组治疗前后血压水平均无明显变化。 FMD与脂联素水平(β=0.308, P <0.05)和NO水平(β=0.417, P <0.05)均呈正相关。结论缬沙坦可能通过增加冠心病患者血浆中脂联素水平改善血管内皮功能。

목적:탐토힐사탄대관심병환자지련소화혈관내피공능적영향。방법장40례관심병환자수궤분위2조,상규치료조20례급여아사필림화주석산미탁락이(배타악극),힐사탄조20례급여상규치료가힐사탄。치료전화치료8주후,채용초성다보륵법측정혈류개도적굉동맥서장반응( FMD),병검측치료전후환자혈장지련소수평화일양화담( NO)수평。결과여치료전비교,상규치료조굉동맥기출내경、FMD、NO、지련소수평균무현저변화(P >0.05),힐사탄조제굉동맥기출내경외,기여지표균현저승고( P<0.05, P <0.01)。여상규치료조상비,힐사탄조치료후FMD、지련소、NO수평균명현증가( P <0.05, P <0.01)。2조치료전후혈압수평균무명현변화。 FMD여지련소수평(β=0.308, P <0.05)화NO수평(β=0.417, P <0.05)균정정상관。결론힐사탄가능통과증가관심병환자혈장중지련소수평개선혈관내피공능。
Objective To investigate the effects of valsartan on adiponectin and endothelial function in patients with coronary heart disease .Methods Forty patients were randomly divided into 2 groups, the conventional treatment group of 20 cases treated with aspirin and tartaric acid metoprolol (Betaloc), valsartan group of 20 cases were given conventional treatment and valsartan .Before and 8 weeks after treatment , ultrasonic Doppler examination were performed for the determination of bra-chial artery flow mediated vasodilation ( FMD) before and after treatment , and patients'plasma adiponectin levels and nitric oxide (NO) level.Results Compared with before treatment, the conventional treatment group of the brachial artery diameter , FMD, NO, adiponectin level had no significant changes ( P >0.05), valsartan group except brachial arteries , other indexes were significantly elevated ( P <0.05, P <0.01).Compared with the conventional treatment group , after treatment, valsar-tan group's FMD, adiponectin, NO levels were significantly increased ( P <0.05, P <0.01).Before and after treatment, blood pressure levels in the 2 groups had no obvious change .FMD and adiponectin levels (β=0.308, P <0.05) and NO (β=0.417, P <0.05) were positively correlated.Conclusion The valsartan could improve endothelial function in patients with coronary heart disease by increasing adiponectin levels .
