描述肝癌患者行肝动脉栓塞化疗(transcatheter arterial chemoemoembolization,TACE)术后自我感受负担(self-perceived burden,SPB)的真实感受,分析患者 SPB 的影响因素。方法采用质性研究中的现象学研究方法,对某三级甲等传染病医院12例 TACE 术后肝癌患者进行深入访谈,采用以 Colaizzi 7步分析法对资料进行分析、归纳、提炼。结果通过阅读、分析、思考和提炼,得出4个主题：(1)负担最重的是担心照顾者身心疲惫；(2)担心给家庭造成经济负担；(3)担心自己的疾病得不到预期治疗效果；(4)心理负担过重。结论TACE 术后患者都存在不同程度的自我感受负担,医护人员应重视患者因 SPB 而引起的一系列心理问题,提供心理护理,给予心理疏导。

To describe the self-perceived burden(SPB)in liver cancer patients accepted tran-scatheter arterial Chemoemoembolization (TACE)treatment and analyze the influencing factors of SPB. Methods By using the qualitative phenomenological research methodology,12 patients were selected and the in-depth interview were also conducted.The results were analyzed,concluded and refined with Colaizzz’ s seven steps of phenomenological analysis.Results By means of reading,analysis,reflection and refining,4 themes were addressed:(1)fear of cargiver’s mental and physical exhaustion;(2)fear of heavy economy burden;(3)fear of treatment uncertainty;(4)heavy mental burden of themselves.Conclusion Various SPB in liver cancer patients who receive TACE treatment were obvious,and medical staff should pay more at-tention to the SPB in patients and provide them with personalized,high-quality mental nursing and psycho-logical counseling.
