目的：探讨老年重症心力衰竭急诊内科治疗效果。方法选择老年重症心力衰竭患者85例，随机分为观察组(43例)和对照组(42例)。对照组给予常规治疗，观察组在常规治疗基础上给予厄贝沙坦联合氢氯噻嗪治疗。观察两组治疗前和治疗后左室射血分数及B型脑钠肽改变情况。结果观察组治疗后的左室射血分数和B型脑钠肽水平分别与对照组治疗后的左室射血分数和B型脑钠肽水平比较，差异有统计学意义(P<0.05)。观察组总有效率95.3%高于对照组73.8%，差异有统计学意义(P<0.05)。结论厄贝沙坦联合氢氯噻嗪在老年重症心力衰竭急诊治疗中疗效显著，值得借鉴。

Objective To investigate therapeutic effect of elderly patients with severe heart failure in emergency internal medicine. Methods Selected 85 elderly patients with severe heart failure patients in our hospital and randomly divided into observation group and control group. The control group was given conventional treatment, the observation group was given irbesartan combined with hydrochlorothiazide on the base of the conventional treatment. The left ventricular ejection fraction and B-type natriuretic peptide changes were observed before and after treatment. Results The left ventricular ejection fraction and B-type natriuretic peptide level in observation group compared with those in control group after treatment, the difference was statistically significant(P<0.05). The total effective rate in observation group(95.3%) was higher than that in control group(73.8%), the difference was statistically significant(P<0.05). Conclusion The combination of irbesartan and hydrochlorothiazide in the treatment of elderly patients with severe heart failure has significant effect in emergency and the therapy is worthy learning.
