目的：探讨难治心衰病人的病因及临床治疗方法。方法我院心内科2012年12月至2013年12月共收治难治性心衰病人50例，在常规给予休息、吸氧、限盐、限水，纠正贫血、电解质紊乱，控制感染，控制原发病的基础上，联合使用 acei、β受体阻滞剂、利尿剂、醛固酮受体拮抗剂、血管扩张剂、正性肌力药等药物治疗。结果经治疗50例患者显效29例，有效17例，无效4例。结论难治性心衰发病机制较为复杂，在治疗时应充分认识心衰难治的原因，积极处理可纠正的心衰病因和诱因；本院在常规基础治疗的基础上，联合使用 acei、β受体阻滞剂、利尿剂、醛固酮受体拮抗剂、血管扩张剂、正性肌力药等药物治疗，患者心衰症状迅速得到控制，临床效果显著。
목적：탐토난치심쇠병인적병인급림상치료방법。방법아원심내과2012년12월지2013년12월공수치난치성심쇠병인50례，재상규급여휴식、흡양、한염、한수，규정빈혈、전해질문란，공제감염，공제원발병적기출상，연합사용 acei、β수체조체제、이뇨제、철고동수체길항제、혈관확장제、정성기력약등약물치료。결과경치료50례환자현효29례，유효17례，무효4례。결론난치성심쇠발병궤제교위복잡，재치료시응충분인식심쇠난치적원인，적겁처리가규정적심쇠병인화유인；본원재상규기출치료적기출상，연합사용 acei、β수체조체제、이뇨제、철고동수체길항제、혈관확장제、정성기력약등약물치료，환자심쇠증상신속득도공제，림상효과현저。
Objective to investigate the etiology and clinical treatment of patients with refractory heart failure. Method department of cardiology in our hospital in 2012 december -2013 year in december were treated 50 patients with refractory heart failure, give rest, oxygen inhalation, salt, water restrictions in the conventional, correction of anemia, electrolyte disturbances, infection control, basic control of primary disease, the combined use of acei, beta blockers, diuretics, aldosterone receptor antagonist, vascular dilatation agent, positive inotropic drug drug treatment. Result after treatment, 50 cases of 29 patients were cured, effective 17 cases, invalid in 4 cases. Conclusion the mechanism of refratory heart failure is more complex, the reason should be fully aware of heart failure refractory to treatment, active treatment can correct the failure causes and predisposing factors; the school based on the conventional foundation treatment, the combined use of acei, beta blockers, aldosterone antagonists, diuretics, vasodilators, positive inotropic drugs drug treatment, symptoms of heart failure patients under control quickly, the clinical efficacy is significant.
