目的应用正常人新鲜抗凝全血对我院1月份实验室多台血液分析仪进行比对实验,评价各血液分析仪各参数之间是否具有可比性,本次实验研究的是在血液分析过程中临床意义比较大的几个参数:白细胞(WBC)红细胞(RBC)、血红蛋白(HGB)、红细胞压积(HCT)、血小板(PLT)。方法选择本实验室日本Sysmex公司生产的XT一2000i(有配套可溯源的校准物和质控品)血细胞分析仪为参比仪器。先对一正常新鲜抗凝全血定值。用于校准其它血细胞分析仪。校准合格后每日选取一门诊体检人正常新鲜全血在各台仪器上进行测定,以参比仪器所测结果为靶值,其它两台仪器所测结果与参比仪器所测结果相比较,计算出WBC﹑RBC﹑HGB﹑HCT﹑PLT的差异百分比。当差异百分比在允许范围内时表示在控,当差异百分比超出允许范围时表示失控。结果我实验室XT一2000i检测结果与ABX-PENTRA120,迈瑞BC-5500比较,WBC、RBC、HGB、HCT、PLT相关性较好,都在CLIA'88允许误差范围的l/4控制限范围内,实验结果可靠具有可比性。结论同一实验室不同仪器间利用新鲜抗凝全血标本进行比对测定,利用Excel数据点折线图的功能,对差异百分数作图动态观察。及时发现偶然误差和系统误差。查找原因并作相应处理,可以使多台血细胞分析仪的结果间具有可比性,减小差异率,达到了科学又经济的目的。

Objective Applying normal person fresh anticoagulant blood carries out comparison for several hematology analyzers which come from our laboratory in January,and evaluates whether there have been comparability among the parameter of the hematology analyzers.This experiment researches several parameters standing for the great clinical significance in the process of hemanalysis.Methods The Blood Cel Analyzer -XT-2000i (there are the calibrated mat er and the control materials that are assorted and traced to the source) made in SYSMEX corporation of Japan is chosen to be reference instrument.First,fixing numerical value for fresh anticoagulant blood is so as to calibrate another Blood Cell Analyzers.After calibration is qualified,the normal and fresh blood from an outpatient wil be chosen every day in order to be made determination on the instruments.Meanwhile,we regard the result measured by reference instrument as target value,compare it with the measured result of another two instruments and calculate discrepant percentage of WBC,RBC,HGB,HCT,PCT.When discrepant percentage comes within allowed range the result could be under control;when discrepant percentage exceeds al owed range,it could be out of control.Results Comparing the measured result of XT-2000i in the laboratory with one of ABX-PENTRA120,MINDARY BC-5500,WBC,RBC,HGB,HCT,PCT all possess the bet er pertinence,which is in 1/4 control range of permit ed error range of CLIA'88.The laboratory result is so reliable and has comparability.Conclusion Among dif erent instruments in the same laboratory,making use of fresh anticoagulant blood specimen to advance comparison determination and taking advantage of the function of line chart made up of Excel's data points to dynamical y examine discrepant percentage,quickly find accidentalerrorsystematic residuals,ask for reasons in order to carry out the homologous solution could create comparability among the results of several Blood Cel Analyzers,minish discrepant ratio so as to achieve scientific objective and economic purpose.
