目的:探讨左足软组织上皮样血管内皮组织瘤(EH)的临床病理特征,提高对该疾病的诊断、治疗和转归的认识。方法对我院诊治的1例左足软组织上皮样血管内皮细胞瘤的临床资料分析,观察其影像表现、组织病理学、免疫组织化学特征,并结合文献进行讨论。结果左足软组织上皮样血管内皮细胞瘤无典型临床表现以及影像表现,光镜可见:瘤细胞呈短梭形,巢状,索条状,不规则分布于黏液样或透明变性的间质中,可见原始的单细胞血管腔;深部瘤组织细胞丰富,圆形、卵圆形,可见核分裂。免疫组化(Vim+,CD34+,CD31+,第Ⅷ因子相关抗原FⅧRag+)。结论原发于足部软组织的上皮样血管内皮细胞瘤(EH)为罕见的低度恶性肿瘤,主要依据组织病理学和免疫组织化学进行诊断。手术切除是主要的治疗手段。

목적:탐토좌족연조직상피양혈관내피조직류(EH)적림상병리특정,제고대해질병적진단、치료화전귀적인식。방법대아원진치적1례좌족연조직상피양혈관내피세포류적림상자료분석,관찰기영상표현、조직병이학、면역조직화학특정,병결합문헌진행토론。결과좌족연조직상피양혈관내피세포류무전형림상표현이급영상표현,광경가견:류세포정단사형,소상,색조상,불규칙분포우점액양혹투명변성적간질중,가견원시적단세포혈관강;심부류조직세포봉부,원형、란원형,가견핵분렬。면역조화(Vim+,CD34+,CD31+,제Ⅷ인자상관항원FⅧRag+)。결론원발우족부연조직적상피양혈관내피세포류(EH)위한견적저도악성종류,주요의거조직병이학화면역조직화학진행진단。수술절제시주요적치료수단。
Objective To investigate the clinical and pathological features of left foot soft tissue epithelioid he-mangioendothelioma, in order to improve the diagnosis, the understanding of the treatment for the disease and prog-nosis. Methods Analyze the clinical data of a case with left foot soft tissue epithelioid hemangioendothelioma in our hospital, observe its manifestations, histopathological, immunohistochemical features, which were also discussed with review of the literature. Results Left foot soft tissue epithelioid hemangioendothelioma didn′t have typical clinical manifestation and imaging. Optical microscope:the tumor cells were spindle shaped, nest like, strip, with irregular dis-tribution in the myxoid or hyalinized stroma, where primitive single cell of vascular cavity could be seen, deep tissue cells were abundant, round, oval with nuclearfission. Immunohistochemistry (Vim+, CD34+, CD31+, the first factor Ⅷrelated antigen F Ⅷ Rag+). Conclusion Primary epithelioid endothelial cells of the foot soft tissue tumor (EH) is a rare low-grade malignant tumor, mainly based on histopathological and immunohistochemical diagnosis. Surgical resec-tion is the main treatment.
