Pediatric Mesenteric Lymphadenitis%High-frequency Ultrasound%Doppler%diagnosis
目的:探讨小儿肠系膜淋巴结炎高频超声诊断标准化流程方法对60例健康儿童(对照组)与100例肠系膜淋巴结炎患儿(研究组)按1~3岁,4~5岁,6~7岁不同年龄段分析高频超声图像,评估淋巴结部位、纵径、横径、纵横比、彩色血流信号分级及频谱多普勒阻力指数。结果研究组淋巴结多位于右中下腹或脐周,多发成群,大小多在2.0cm以下,可探及血流信号呈树枝状、点状、星状。研究组纵径横径均显著高于对照组(t值分别为27.011、34.145,P值均小于0.001)。两组结内血流信号分级差异均有显著统计学意义(x2=15.732、19.253、8.012, P<0.05)。测及研究组血流阻力指数为(0.54±0.01)。结论患儿肠系膜淋巴结部位、纵横径、结内彩色血流信号及频谱多普勒表现等指标可作为高频彩色多普勒超声诊断小儿肠系膜淋巴结炎的标准。

Objective To research the standardization process of high-frequency color doppler ultrasound diagnosis in pediatric mesenteric lymphadenitis. Methods The characteristic of high-frequency color Doppler ultrasound images were analyzed to 60 healthy children (control group) and 100 children with mesenteric lymphadenitis (study group) which were divided into 1-3 years, 4-5 years and 6-7 years group. The accuracy of longitudinal diameter(L), transverse diameter (S), aspect ratio (L/S), color Doppler blood flow signal classification and pulsed Doppler resistance index were evaluated. Results Mesenteric lymph nodes in the study group almost around the navel and the right lower abdomen. All the nodes distributed liking star dotted, ribbon and dendritic. Compared with the control group, L and S in all age groups of the study group were significantly higher (t values for L and S are 27.011 and 34.145 with P<0.001 respectively). The color Doppler blood flow signal classification also exist significant differences between control group and study group (x2 values are 15.732, 19.253 and 8.012 with P<0.05 respectively) and the plused Doppler resistance index were (0.54±0.01). Conclution Mesenteric lymph nodes of location, longitudinal diameter, transverse diameter, color Doppler blood flow signal classification and pulsed Doppler resistance index could as diagnostic criteria for pediatric mesenteric lymphadenitis with high-frequency color doppler ultrasound.
