目的:探讨对原发性高血压患者采用缬沙坦进行治疗,对其肾血流、肾结构和肾功能的影响。方法:选取在我院就诊的原发性高血压患者61例作为研究对象,所有对象均采用缬沙坦进行治疗,疗程为1年,所有患者均进行彩色多普勒超声检查以检测其肾血流及肾结构,比较分析治疗前后患者的肾血流、肾结构及尿和血β-2微球蛋白水平的改变情况。结果:与治疗前相比,所有对象治疗后收缩压和舒张压明显下降,双肾血流量明显增多,双肾动脉舒张期最低血流速度和收缩期最大流速明显升高,双肾搏动指数和血流阻力指数明显降低,双身动脉管壁厚度明显变薄,双肾动脉内径明显增大,且尿液和血液中β2-微球蛋白明显降低(P<0.05)。结论:对原发性高血压患者采用缬沙坦进行治疗,不但能较好地控制患者的血压,而且能明显降低患者肾血管阻力,改善患者肾血流量和肾血管结构,提高患者肾小球滤过率,从而降少尿蛋白的排泄,进而保护患者的肾功能。

목적:탐토대원발성고혈압환자채용힐사탄진행치료,대기신혈류、신결구화신공능적영향。방법:선취재아원취진적원발성고혈압환자61례작위연구대상,소유대상균채용힐사탄진행치료,료정위1년,소유환자균진행채색다보륵초성검사이검측기신혈류급신결구,비교분석치료전후환자적신혈류、신결구급뇨화혈β-2미구단백수평적개변정황。결과:여치료전상비,소유대상치료후수축압화서장압명현하강,쌍신혈류량명현증다,쌍신동맥서장기최저혈류속도화수축기최대류속명현승고,쌍신박동지수화혈류조력지수명현강저,쌍신동맥관벽후도명현변박,쌍신동맥내경명현증대,차뇨액화혈액중β2-미구단백명현강저(P<0.05)。결론:대원발성고혈압환자채용힐사탄진행치료,불단능교호지공제환자적혈압,이차능명현강저환자신혈관조력,개선환자신혈류량화신혈관결구,제고환자신소구려과솔,종이강소뇨단백적배설,진이보호환자적신공능。
To study the effect of Valsartan on the renal blood flow,renal texture and renal function of the patients with primary hypertension.Methods:A total of 61 patients with primary hypertension were enrolled into this study in our hospital.All the subjects were treated with Valsartan for 1 year.All the subjects were received color duplex doppler ultrasound in order to observe the renal blood flow,renal texture and renal function.The following parameters before and after treatment were compared and analyzed:the renal blood flow,renal texture and renal function.Results:Compared with those before treatment,the systolic and diastolic blood pressure of all the subjects after treatment were significantly deceased,the renal blood flow significantly higher,the minimum speed of the renal blood flow during artery diastolic and the maximum speed of the renal blood flow during artery systolic significantly higher,the resistance index and pulsatility index of the double renal significantly deceased,the vessel wall thickness of the artery significantly thin,the artery diameter of the double kidney significantly big,and the levels of the urine and bloodβ2-microglobulin significantly deceased.Conclusion:For the patients with primary hypertension,the Valsartan can significantly deceased the systolic and diastolic blood pressure,resistance index,and the urine and bloodβ2-microglobulin level,and it also can significantly improve the the renal blood flow,renal texture,glomerular filtration rate. There fore,it can be helpful to protect the renal function of the patients with primary hypertension.
