目的：探讨哌拉西林／他唑巴坦和头孢米诺治疗急性脑卒中患者医院获得性肺炎临床效果，以期提高临床诊治水平。方法选取2010-01-2013-01130例急性脑卒中并医院获得性肺炎患者为研究对象，分成2组，在综合治疗基础上对照组予以头孢米诺治疗，观察组予以哌拉西林／他唑巴坦治疗，观察2组临床效果、临床症状体征消失时间、病原菌清除情况和安全性。结果2组在临床效果、病原菌清除率和药物安全性上比较差异均无统计学意义（ P＞0.05）；但在住院时间、咳嗽、咯黄脓痰、发热、肺部啰音、X线影像、末梢血象恢复时间上比较差异有统计学意义（ P均＜0.05）。结论哌拉西林／他唑巴坦治疗急性脑卒中并医院获得性肺炎能缩短病程，减少住院时间，无明显不良反应。
목적：탐토고랍서림／타서파탄화두포미낙치료급성뇌졸중환자의원획득성폐염림상효과，이기제고림상진치수평。방법선취2010-01-2013-01130례급성뇌졸중병의원획득성폐염환자위연구대상，분성2조，재종합치료기출상대조조여이두포미낙치료，관찰조여이고랍서림／타서파탄치료，관찰2조림상효과、림상증상체정소실시간、병원균청제정황화안전성。결과2조재림상효과、병원균청제솔화약물안전성상비교차이균무통계학의의（ P＞0.05）；단재주원시간、해수、각황농담、발열、폐부라음、X선영상、말소혈상회복시간상비교차이유통계학의의（ P균＜0.05）。결론고랍서림／타서파탄치료급성뇌졸중병의원획득성폐염능축단병정，감소주원시간，무명현불량반응。
Objective To observe the clinical effect of Piperacillin/Tazobactam and cefminox in the treatment hospital-ac-quired pneumonia patients with acute stroke ,and to improve the clinical diagnosis level .Methods 130 cases with acute stroke combined with hospital-acquired pneumonia treated in our hospital from January 2010 to January 2013 were selected as the re-search objects and were divided into two groups .Based on comprehensive treatment ,the control group was treated with cefmi-nox ,and the observation group was treated with Piperacillin/Tazobactam .The clinical effect ,the die-out time of clinical symp-toms ,pathogen clear and safety of two groups after treatment were observed .Results There were no significant differences between two groups for clinical effect ,clearance rate of clinical symptoms and safety , P> 0 .05 ,but there were significant differences for length of stay ,cough ,sputum ,fever ,lung rale ,X-ray photograph and recovery time of peripheral hemogram , P<0 .05 .Conclusion Piperacillin/Tazobactam in the treatment of acute stroke combined with hospital-acquired pneumonia , not only can reduce the course of disease and the length of stay ,but also has no evident adverse reactions .
