目的：探讨加味四妙汤对急性痛风性关节炎患者血清白介素-6(IL-6)、白介素-8(IL-8)和基质金属蛋白酶-3(MMP-3)水平的影响及疗效观察.方法将2010年1月~2013年7月浙江省嘉兴市中医院骨伤科门诊84例急性痛风性关节炎患者随机分为观察组和对照组,每组42例.两组患者均予以饮食控制和多饮水等常规处理,并加用秋水仙碱口服,首剂1 mg,之后0.5 mg/次,每2小时1次,24 h后改0.5 mg,3次/d.观察组在此基础上患者加用加味四妙汤,每日1剂,水煎服,连用14 d.比较两组患者治疗前后血清IL-6、IL-8和MMP-3水平的变化,并进行疗效观察.结果治疗14 d后,两组患者血清IL-6、IL-8和MMP-3水平[(48.26±10.76)ng/L、(1.32±0.32)μg/L和(81.55±20.52)ng/mL；(34.37±8.13)ng/L、(0.89±0.25)μg/L和(64.28±13.19)ng/mL]均较治疗前明显下降[(70.19±16.12)ng/L、(1.73±0.43)μg/L和(103.92±27.16)ng/mL；(69.52±14.02)ng/L、(1.68±0.34)μg/L和(99.74±26.19)ng/mL](t =2.45、2.27、2.19、3.25、3.09、2.91,P<0.01或P<0.05),且观察组下降值比对照组更明显(t =2.34、2.43、2.29,P<0.05)；且观察组总有效率明显高于对照组(92.86%比76.19%)(字2=4.46,P<0.05).结论加味四妙汤治疗急性痛风性关节炎具有较好效果,能明显改善患者的症状及体征,有助于关节功能的恢复,作用与其降低血清IL-6、IL-8和MMP-3水平以及抑制局部炎症反应过程密切有关。

목적：탐토가미사묘탕대급성통풍성관절염환자혈청백개소-6(IL-6)、백개소-8(IL-8)화기질금속단백매-3(MMP-3)수평적영향급료효관찰.방법장2010년1월~2013년7월절강성가흥시중의원골상과문진84례급성통풍성관절염환자수궤분위관찰조화대조조,매조42례.량조환자균여이음식공제화다음수등상규처리,병가용추수선감구복,수제1 mg,지후0.5 mg/차,매2소시1차,24 h후개0.5 mg,3차/d.관찰조재차기출상환자가용가미사묘탕,매일1제,수전복,련용14 d.비교량조환자치료전후혈청IL-6、IL-8화MMP-3수평적변화,병진행료효관찰.결과치료14 d후,량조환자혈청IL-6、IL-8화MMP-3수평[(48.26±10.76)ng/L、(1.32±0.32)μg/L화(81.55±20.52)ng/mL；(34.37±8.13)ng/L、(0.89±0.25)μg/L화(64.28±13.19)ng/mL]균교치료전명현하강[(70.19±16.12)ng/L、(1.73±0.43)μg/L화(103.92±27.16)ng/mL；(69.52±14.02)ng/L、(1.68±0.34)μg/L화(99.74±26.19)ng/mL](t =2.45、2.27、2.19、3.25、3.09、2.91,P<0.01혹P<0.05),차관찰조하강치비대조조경명현(t =2.34、2.43、2.29,P<0.05)；차관찰조총유효솔명현고우대조조(92.86%비76.19%)(자2=4.46,P<0.05).결론가미사묘탕치료급성통풍성관절염구유교호효과,능명현개선환자적증상급체정,유조우관절공능적회복,작용여기강저혈청IL-6、IL-8화MMP-3수평이급억제국부염증반응과정밀절유관。
Objective To discuss influence and curative effect observation of Jiawei Simiao Decoction on serum inter-leukin-6 (IL-6) and interleukin-8 (IL-8) and matrix metalloprotease-3 (MMP-3) levels of patients with acute gouty arthritis. Methods 84 cases of patients with acute gouty arthritis, who were in Department of Orthopedics and Trauma-tology of Traditional Chinese Medicine Hospital of Jiaxing City from January 2010 to July 2013 were divided into ob-servation group and control group at random, with 42 cases respectively. The patients in two groups were given routine treatment like diet control, drinking plenty of water and etc, and were additionally given colchicin through the mouth, with the first dose of 1 mg, 0.5 mg for every 2 hours later, and 0.5 mg after 24 hours for three times a day. The patients in observation group were additionally given Jiawei Simiao Decoction one dose a day, which was taken decocted with water for 14 days. The changes of serum IL-6 and IL-8 and MMP-3 levels of patients in two groups before and after medical treatment were compared, and curative effect observation was carried on as well. Results After 14 days' medi-cal treatment, serum IL-6 and IL-8 and MMP-3 levels of patients in two groups [(48.26±10.76) ng/L, (1.32±0.32) μg/L and (81.55±20.52) ng/mL; (34.37±8.13) ng/L, (0.89±0.25) μg/L and (64.28±13.19) ng/mL] obviously declined than be-fore [(70.19±16.12) ng/L, (1.73±0.43)μg/L and (103.92±27.16) ng/mL;(69.52±14.02) ng/L, (1.68±0.34)μg/L and (99.74±26.19) ng/mL] (t=2.45, 2.27, 2.19, 3.25, 3.09, 2.91, P<0.01 or P<0.05), and the declining rate in observation group was much higher than that in control group (t = 2.34, 2.43, 2.29, P< 0.05). The total clinical efficiency of patients in observation group was much higher than that in control group (92.86%v s 76.19%) (χ2=4.46, P<0.05). Conclusion Ji-awei Simiao Decoction has reliable curative effect on patients with acute gouty arthritis, can obviously improve the symptom and physical signs and it is favorable for the recovery of function of joint, whose mechanism of action has close effect on reducing serum IL-6, IL-8 and MMP-3 levels and inhibiting local inflammatory process.
