目的:观察2型糖尿病患者血清同型半胱氨酸( HCY)、脂联素( APN)水平的变化并探讨其临床意义。方法选取25例健康体检者为对照组,65例2型糖尿病患者为糖尿病组,检测2组血清 HCY、APN、空腹血糖( FPG)、空腹胰岛素( FINS)、总胆固醇( TC)、三酰甘油(TG)及游离脂肪酸(FFA)的水平并计算胰岛素抵抗指数(HOMA - IR)并进行比较,同时分析其相关性。结果糖尿病组血清 HCY 水平及 FPG、HOMA - IR、TC、TG、FFA 水平显著高于对照组(P ﹤0.05或0.01),APN 水平显著低于对照组( P ﹤0.01),糖尿病组 HCY≥15μmol / L 者 APN 水平显著低于 HCY ﹤15μmol / L 者。消除 FPG、HOMA - IRI、TC、TG 因素影响,糖尿病组的 HCY 水平与 ADP 呈显著负相关。结论2型糖尿病患者 HCY 水平高于正常对照组,APN 水平低于正常对照组,HCY 与 APN 呈负相关,高 HCY 与低 APN 水平可能参与了胰岛素抵抗及糖尿病的发生、发展。

목적:관찰2형당뇨병환자혈청동형반광안산( HCY)、지련소( APN)수평적변화병탐토기림상의의。방법선취25례건강체검자위대조조,65례2형당뇨병환자위당뇨병조,검측2조혈청 HCY、APN、공복혈당( FPG)、공복이도소( FINS)、총담고순( TC)、삼선감유(TG)급유리지방산(FFA)적수평병계산이도소저항지수(HOMA - IR)병진행비교,동시분석기상관성。결과당뇨병조혈청 HCY 수평급 FPG、HOMA - IR、TC、TG、FFA 수평현저고우대조조(P ﹤0.05혹0.01),APN 수평현저저우대조조( P ﹤0.01),당뇨병조 HCY≥15μmol / L 자 APN 수평현저저우 HCY ﹤15μmol / L 자。소제 FPG、HOMA - IRI、TC、TG 인소영향,당뇨병조적 HCY 수평여 ADP 정현저부상관。결론2형당뇨병환자 HCY 수평고우정상대조조,APN 수평저우정상대조조,HCY 여 APN 정부상관,고 HCY 여저 APN 수평가능삼여료이도소저항급당뇨병적발생、발전。
Objective It is to observe the changes of serum levels of homocysteine(HCY)and adiponectin(APN)in pa-tients with type 2 diabetes mellitus(T2DM),and approach its’clinical significance. Methods 65 patients with T2DM were selected to T2DM group and 25 healthy people to control group. The levels of serum HCY,APN,fasting plasma glucose ( FPG),fasting insulin(FINS),total cholesterol(TC),triglyceride(TG),free fatty acid(FFA)were simultaneously meas-ured,and homeostatic model assessment for insulin resistance(HOMA - IR)was calculated according to FPG and FINS. All the serum predictors were compared between two groups,and their correlation was analyzed. Results Serum levels of HCY, FPG,HOMA - IR,TG,TC and FFA in T2DM group were significantly higher than those in control group,but serum level of APN in T2DM group was significantly decreased compared with control group. Serum levels of APN in HCY ≥15 μmol / L group was significantly decreased as compared with HCY ﹤ 15 μmmol / L group. Serum level of HCY was negatively correlated with APN in T2DM group after the influence of FPG,HOMA - IR,TG,TC were corrected in the partial correlation analysis. Conclusion In T2DM group serum levels of HCY is increased,but serum levels of APN is decreased,the serum level of HCY is negatively correlated with APN,higher serum levels of HCY and lower serum levels of APN are related to the process of in-sulin resistance and T2DM.
