目的:分析结核抗体检测在小儿结核病诊断中的应用效果。方法选择本地区2012-2013年收治的结核病患儿患48例为观察组,并以同期入院的非结核病患儿35例为对照组,并对两组患儿的体液标本涂片、卡介菌纯化蛋白衍生物(PPD)和结核抗体检测结果进行比较。结果三种方法组间差异具有统计学意义,其中以PPD法阳性率最高达68.8%,涂片法最低(27.0%);不同类型结核检测结果方面:涂片法肺结核的阳性率显著高于肺外结核(P<0.05),而PPD法和结核抗体法在肺结核和肺外结核的阳性率方面差异均不显著(P>0.05)。结论结核抗体检测、涂片法和PPD检测在结核病诊断中各有特色,其中结核抗体检测尤其适于作为儿童、肺外结核及无痰患者的快速辅助诊断手段。

Objective To investigate the application of tuberculosis antibody in diagnosis of children with tuberculosis. Methods 48 patients with tuberculosis of our area from 2012 to 2013 were selected as observation group, and other 35 patients without tu-berculosis were as control, and the results of humoral smear, PPD and tuberculosis antibody detection were compared of two groups. Results There were significantly difference between three methods, and the positive rate of PPD was 68.8%,higher than humoral smear(27.0%); Results of different types of TB:the positive rate of tuberculosis were higher than extrapulmonary tubercu-losis by humoral smear(P<0.05), and there was no difference between PPD and tuberculosis antibody method in diagnosis of differ-ent types of TB. Conclusion Humoral smear, PPD and tuberculosis antibody detection all have its virtue in the diagnosis of tuber-culosis, and the tuberculosis antibody detection especially fit as a quick diagnosis tool for children, extrapulmonary tuberculosis and patients without sputum.
