目的:比较初发2型糖尿病(T2DM)不同糖化血红蛋白(HbA1c)水平与血糖、C肽、胰岛素分泌功能等的关系。方法:选取我院2012年9月~2013年9月期间内分泌门诊新诊断的T2DM患者110例,男67例,女43例;年龄38~76岁,平均(53.2±3.4)岁。按HbA1c水平分为三组,同时选取健康人30例(男17例,女13例;年龄36~73岁,平均(46.2±3.4)岁)作为对照。所有入组对象行口服葡萄糖耐受试验(OGTT)试验及胰岛素释放试验,取空腹及负荷后120min静脉血,测定HbA1c、空腹血糖(FPG)、负荷后120min血糖(2hPG)、空腹C肽(FCP)、负荷后120minC肽(2hCP)、空腹胰岛素(FIns)、负荷后120min胰岛素(2hIns)。计算HOMA-IR及HOMA-β。结果:(1)FPG、2hPG随HbA1c的升高而升高。(2)T2DM患者与健康人相比,2hCP、2hIns 呈现先升高后降低的趋势,具体表现为HbA1c≤10%时,2hCP、2hIns升高,HbA1c>10%时,2hCP、2hIns降低。(3)与健康人相比,T2DM患者HOMA-IR显著增高,不同水平HbA1c、HOMA-IR无明显差异。(4)T2DM患者HbA1c与FPG、2hPG呈显著正相关,与CP、2hCP、FIns、2hIns、HOMA-β呈显著负相关。结论:初发T2DM患者进行OGTT试验及胰岛素释放试验,可反映HbA1c水平与胰岛分泌功能的相关性,可为临床用药提供参考。

목적:비교초발2형당뇨병(T2DM)불동당화혈홍단백(HbA1c)수평여혈당、C태、이도소분비공능등적관계。방법:선취아원2012년9월~2013년9월기간내분비문진신진단적T2DM환자110례,남67례,녀43례;년령38~76세,평균(53.2±3.4)세。안HbA1c수평분위삼조,동시선취건강인30례(남17례,녀13례;년령36~73세,평균(46.2±3.4)세)작위대조。소유입조대상행구복포도당내수시험(OGTT)시험급이도소석방시험,취공복급부하후120min정맥혈,측정HbA1c、공복혈당(FPG)、부하후120min혈당(2hPG)、공복C태(FCP)、부하후120minC태(2hCP)、공복이도소(FIns)、부하후120min이도소(2hIns)。계산HOMA-IR급HOMA-β。결과:(1)FPG、2hPG수HbA1c적승고이승고。(2)T2DM환자여건강인상비,2hCP、2hIns 정현선승고후강저적추세,구체표현위HbA1c≤10%시,2hCP、2hIns승고,HbA1c>10%시,2hCP、2hIns강저。(3)여건강인상비,T2DM환자HOMA-IR현저증고,불동수평HbA1c、HOMA-IR무명현차이。(4)T2DM환자HbA1c여FPG、2hPG정현저정상관,여CP、2hCP、FIns、2hIns、HOMA-β정현저부상관。결론:초발T2DM환자진행OGTT시험급이도소석방시험,가반영HbA1c수평여이도분비공능적상관성,가위림상용약제공삼고。
Objective:To compare the onset of type 2 diabetes mel itus (T2DM) different glycosylated hemoglobin (HbA1c) levels and blood glucose, C-peptide and insulin secretion function relationships. Methods:Selected the 110 patients with newly diagnosed T2DM from endocrine clinic in Our hospital from September 2012 to September 2013, 67 males, 43 females;aged 38~76 years, mean (53.2±3.4) years of age. Divided into three groups according to the level of HbA1c,Meanwhile selected 30 cases(17 males, 13 females;aged 36~73 years, mean (46.2±3.4) years old) of healthy people as controls. Al group objects into line oral glucose tolerance test (OGTT) test and insulin release test,took blood Fasting and afterload 120min, Determined HbA1c,FPG,2hPG,FCP,2hCP,Fins,2hIns, Calculated HOMA-IR and HOMA-β. Results:(1) FPG, 2hPG increases with the rise of HbA1c. (2) T2DM patients compared with healthy people, 2hCP, 2hIns showed ifrst increased and then decreased, the speciifc performance of HbA1c≤10%when, 2hCP, 2hIns elevated, HbA1c>when 10%, 2hCP, 2hIns reduced. (3) Compared with healthy people, T2DM patients were signiifcantly higher HOMA-IR, different levels of HbA1c, HOMA-IR was no signiifcant difference. (4) T2DM patients with HbA1c and FPG, 2hPG was signiifcantly positively correlated with CP, 2hCP, FIns, 2hIns, HOMA-βwas signiifcantly negatively correlated. Conclusion:The onset T2DM patients OGTT test and insulin release test can relfect the level of correlation between HbA1c and insulin secretion, can provide a reference for clinical use.
