目的:观察无创正压通气对急性肺血栓栓塞症(PTE)患者心、肺功能及血流动力学的影响。方法:32例接受无创正压通气治疗的急性PTE患者,超声心动图监测上机前后血流动力学及心功能参数,血气分析仪测定治疗前后动脉血气指标。结果:无创通气治疗后,患者心率、呼吸频率下降(P=0.001),PaO2与P(A-a)O2显著改善(P=0.001)。动脉收缩压与舒张压稳定(P>0.05),三尖瓣反流压差、肺动脉收缩压、右室舒张末期横径、左室舒张末期直径、左室每搏量均无显著变化(P>0.05)。右室前壁活动度有轻度增加趋势,无统计学意义(P=0.06)。结论:无创正压通气可显著改善急性PTE患者的氧合弥散功能,对血流动力学及心功能未见显著影响。

목적:관찰무창정압통기대급성폐혈전전새증(PTE)환자심、폐공능급혈류동역학적영향。방법:32례접수무창정압통기치료적급성PTE환자,초성심동도감측상궤전후혈류동역학급심공능삼수,혈기분석의측정치료전후동맥혈기지표。결과:무창통기치료후,환자심솔、호흡빈솔하강(P=0.001),PaO2여P(A-a)O2현저개선(P=0.001)。동맥수축압여서장압은정(P>0.05),삼첨판반류압차、폐동맥수축압、우실서장말기횡경、좌실서장말기직경、좌실매박량균무현저변화(P>0.05)。우실전벽활동도유경도증가추세,무통계학의의(P=0.06)。결론:무창정압통기가현저개선급성PTE환자적양합미산공능,대혈류동역학급심공능미견현저영향。
Objective:To investigate the inlfuence of hemodynamics and cardiac function of noninvasive mechanical ventilation on patients with acute pulmonary thromboembolism(PTE). Methods:32 patients with acute PTE were included. All of them were treated by noninvasive ventilator. Hemodynamics and cardiac function were detected through Doppler echocardiography before and after six hours treatment. Arterial blood gas was monitored at the same time. Results:Heart rates and respiratory rates of patients lowered signiifcantly after six hours treatment of noninvasive ventilation(P=0.001). PaO2 and P(A-a)O2 improved at the same time(P=0.001). While systolic and diastolic arterial pressure and the cardiac function had no notable change.Conclusions:Noninvasive mechanical ventilation could improved the oxygenation and diffusion function of patients with acute PTE efifciently, no remarkable inlfuence of hemodynamics and cardiac function was found.
