目的:探讨APACHE-Ⅳ危重症评分系统在心血管疾病的应用。方法:计算1005名阜外心血管病医院ICU的心血管重症患者每人的死亡危险性、预测入住ICU时间(LOS)和APACHE-Ⅳ分值,按疾病种类和分值分组,比较各组的实际病死率、预测病死率、APACHE-Ⅳ分值、预测住院时间和实际住院时间。结果:ROC曲线下的面积为0.764。实际病死率随APACHE-Ⅳ分值增加而增加。95%可信区间:冠心病组,瓣膜病组、心肌病组、<30分组、>60分组、有CABG手术史组预测病死率在实际病死率内。预测LOS和实际LOS相关,差异明显。结论:对于预测心血管疾病病死率,APACHE-Ⅳ的价值有限。

Objective:To explore the application of APACHE-IV critical illness scoring system in cardiovascular disease. Methods:The calculation of 1005 ICU patients with severe Fuwai Hospital of Cardiovascular Disease cardiovascular death risk of each prediction, ICU stay time (LOS) and the APACHEⅣscore, grouped according to the type of diseases and the actual score, mortality rate, compared to predict mortality rate, APACHE-Ⅳscore, prediction of hospitalization time and the actual time of hospitalization. Results:The area under the ROC curve was 0.764. The actual mortality rate increases with increasing APACHE-Ⅳvalue. 95%conifdence interval:the group of coronary heart disease, valvular heart disease, cardiomyopathy group,<30 group,>60 group, CABG group operation history prediction case fatality rate in the actual mortality rate in. Correlation of calculate LOS and actual LOS, signiifcant difference. Conclusions:For the prediction of cardiovascular disease mortality rate, the value of APACHE-Ⅳis ifnite.
