目的：探讨No．6组淋巴结活检在胃食管结合部腺癌全胃切除中的意义。方法行近端胃切除能达到根治的胃食管结合部腺癌病例，术中切除No．6组淋巴结送术中快速冰冻切片病理学检查，如淋巴结转移则行全胃切除，反之行近端胃切除。结果372例中，No．6组淋巴结活检阳性32例，总阳性率8．60％，且随着肿瘤浸润深度增加No．6组淋巴结转移率明显增加。结论 No．6组淋巴结有一定的转移比例，当No．6组淋巴结转移需行全胃切除。 No．6组淋巴结术中冰冻切片病理诊断在指导胃食管结合部腺癌手术切除范围中具有重要意义。
목적：탐토No．6조림파결활검재위식관결합부선암전위절제중적의의。방법행근단위절제능체도근치적위식관결합부선암병례，술중절제No．6조림파결송술중쾌속빙동절편병이학검사，여림파결전이칙행전위절제，반지행근단위절제。결과372례중，No．6조림파결활검양성32례，총양성솔8．60％，차수착종류침윤심도증가No．6조림파결전이솔명현증가。결론 No．6조림파결유일정적전이비례，당No．6조림파결전이수행전위절제。 No．6조림파결술중빙동절편병리진단재지도위식관결합부선암수술절제범위중구유중요의의。
Objective To study the value of group 6 lymph node biopsy in total gastrectomy for gastroesophageal junction adenocarcinoma .Methods Patients treated with proximal gastrectomy who achieved radical effect were selected .group 6 lymph nodes frozen pathologic detection was performed during operation .Total gastrectomy was carried out if group 6 lymph node metas-tasis exited,otherwise,the proximal gastrectomy was performed .Results 32 cases of group 6 lymph node biopsy-positive was de-tected in total 372 cases.The total positive rate was 8.60%,with the increasing of depth of tumor invasion ,metastasis rate in-creased.Conclusion Group 6 lymph node metastasis has a certain proportion ,group 6 lymph node metastases require total gas-trectomy to achieve radical results .Group 6 lymph node biopsy has important significance in guiding gastroesophageal junction ad -enocarcinoma resection .
