目的探讨嗓音障碍指数(voice handicap index,VHI)评估职业用声者嗓音障碍程度的效能及临床意义。方法分别对60例有声带增生性病变和60例无声带增生性病变的职业用声者进行VHI调查,VHI调查包括功能(F)、生理(P)、情感(E)3个范畴,记录三方面得分及总分(TVH)分值,与每日说话时间、病程和有无用力发声行为进行相关性分析,并与60例正常对照组进行对比。结果有声带增生病变组VHI的F、P、E、TVH平均值分别为(9.41±7.21)、(18.25±8.86)、(8.42±7.92)、(35.32±21.18)分,无声带增生病变组VHI的F、P、E、TVH平均值分别为(4.57±2.62)、(7.86±3.91)、(3.97±3.77)、(15.83±9.32)分,正常组的VHI的F、P、E、TVH平均值分别为(0.65±0.83)、(0.92±0.82)、0、(1.47±1.42)分。有声带增生病变组和无声带增生病变组的VHI平均值均高于对照组(P<0.05),有声带增生病变组VHI平均值均高于无声带增生病变组(P<0.05)。有声带增生病变组VHI均与每日说话时间、病程和用力发声行为存在相关性(r分别为0.434、0.462、0.402,P均<0.05)。无声带增生病变组VHI均与每日说话时间存在相关性(r=0.413,P<0.05),与病程及用力发声行为无相关性(r分别为0.132和0.213,P>0.05)。结论对职业用声者进行VHI评估有利于其认识自身嗓音的状态,每日说话时间过长可加重职业用声者的嗓音障碍程度。

Objective To observe the psychosocial impact of dysphonia on careers with occupational voice with voice handicap index(VHI). Methods VHI was administered to the proliferative lesion group(60cases),the non-proliferative lesion group (60cases) and the control group (60cases). VHI scale included functional(F),physiological (P),emotional (E),and TVH denoted the total sum. Then correlation between VHI and speaking time per day,duration of current complaint and vocal hyperfunction was analyzed. Results VHI scores of the proliferative lesion group and non-proliferative lesion group were significantly greater than the control group(P<0.05). VHI scores of the proliferative lesion group was greater than non-proliferative lesion group(P<0.05). VHI scores was correlated with speaking time per day,duration of current complaint and vocal hyperfunction in the proliferative lesion group (P<0. 05). While VHI scores was correlated with speaking time per day in the non-proliferative lesion group. Conclusion VHI scale can be effectively used to evaluate the severity of voice disorder in careers with occupational voice. The speaking time per day may af-fect VHI in careers with occupational voice.
