目的：观察地五养肝方联合恩替卡韦治疗乙型肝炎病毒相关性肾炎（ HBV-GN）的疗效及其对白介素-2（ IL-2）、γ-干扰素（ IFN-γ）的影响。方法21例HBV-GN患者予以地五养肝方联合恩替卡韦治疗36周，观察患者临床症状、体征，检测患者肝肾功能、24 h尿蛋白定量、HBV DNA水平及血清IL-2、IFN-γ水平等。结果治疗36周后，HBV-GN患者临床症状、体征、肝肾功能较前明显改善（P均＜0．01）；总有效率86％，HBV DNA转阴率81％；血清IL-2、IFN-γ水平较治疗前明显升高（ P均＜0．01）。结论恩替卡韦联合地五养肝方治疗HBV-GN疗效显著，其作用机制可能与高效抗病毒、调节机体的免疫功能及减少肝肾的免疫损伤等有关。
목적：관찰지오양간방연합은체잡위치료을형간염병독상관성신염（ HBV-GN）적료효급기대백개소-2（ IL-2）、γ-간우소（ IFN-γ）적영향。방법21례HBV-GN환자여이지오양간방연합은체잡위치료36주，관찰환자림상증상、체정，검측환자간신공능、24 h뇨단백정량、HBV DNA수평급혈청IL-2、IFN-γ수평등。결과치료36주후，HBV-GN환자림상증상、체정、간신공능교전명현개선（P균＜0．01）；총유효솔86％，HBV DNA전음솔81％；혈청IL-2、IFN-γ수평교치료전명현승고（ P균＜0．01）。결론은체잡위연합지오양간방치료HBV-GN료효현저，기작용궤제가능여고효항병독、조절궤체적면역공능급감소간신적면역손상등유관。
Objective It is to observe the effect of entecavir combined with Diwu Yanggan anagraph ( DWYGA) in treat-ment of hepatitis B virus associated nephritis ( HBV-GN) and its influence on IL-2 and IFN-γ.Methods 21 patients with HBV-GN were received entecavir combined with DWYGA for at least 36 weeks.The changes of clinical symptom and signs , liver and kidney function , 24hUAlb, HBV DNA, IL-2 and IFN-γwere observed.Results Compared with pre -therapy, the clinical symptom and signs, liver and kidney function of patients with HBV -GN were markedly improved (P<0.01). The total effective rate was 86%, HBVDNA negative rate was 81%.The levels of IL-2 and IFN-γwere markedly height-ened than pre-therapy ( P<0.01 ) .Conclusion The combined therapy with entecavir and DWYGA is effective for HBV -GN.The mechanism may be relevant to efficient anti -virus, regulate the body’s immune function, and reduce the damage on kidney and liver .
