目的比较自体封闭式巩膜引流术和Ahmed青光眼阀植入术治疗难治性青光眼的临床疗效.方法对2006年1月至2012年1月在淮南市晨光眼科医院就诊的难治性青光眼患者49只眼分为自体封闭式巩膜引流术组(25只眼)和Ahmed青光眼阀植入术组(24只眼).于术后对两组患者的眼压和手术并发症进行观察.结果自体封闭式巩膜引流术组和Ahmed青光眼阀植入术组术前、术后1周和1个月时眼压比较,差异无统计学意义(P＞0.05).术后6个月时自体封闭式巩膜引流术组眼压显著低于Ahmed青光眼阀植入术组差异有统计学意义(P＜0.05).自体封闭式巩膜引流术组手术并发症的发生率显著低于Ahmed青光眼阀植入术组,差异有统计学意义(P＜0.05).结论自体封闭式巩膜引流术可有效控制难治性青光眼的眼压,且手术并发症较少.

Objective To compare the clinical effects of autologous and enclosed scleral shunt and glaucoma valve implantation in patients with refractory glaucoma.Methods 49 eyes in refractory glaucoma patients were randomized to undergo the autologous and enclosed scleral shunt and the glaucoma valve implantation.Then the intraocular pressure and operative complications were observed postoperatively.Results There were no significant differences between the autologous and enclosed scleral shunt group and the glaucoma valve implantation group in the intraocular pressure preoperatively and 1 week,1 month after operation(P ＞0.05).Compared with the glaucoma valve implantation group,the intraocular pressure in the autologous and enclosed scleral shunt group was significantly decreased at 6 months after operation (P ＜0.05).The rate of operative complications was also significantly decreased postoperatively in the autologous and enclosed scleral shunt group (P ＜0.05).Conclusion The autologous and enclosed scleral shuntcan effectively reduce the intraocular pressure in patients with refractory glaucoma with less complications.
