目的:探讨PFNA微创治疗82例高龄日常行动能力低股骨粗隆间骨折。方法:比较41例高龄日常行动能力低股骨粗隆间骨折经过DHS治疗后和PFNA微创治疗后患者的手术时间,术后出血量、术后出血量、术后疼痛视觉模拟评分法(VAS)以及髋关节Harris评分。结果:选取的41例PFNA微创治疗治疗患者,经过治疗后在患者手术时间,术后出血量、下地时间、术后疼痛视觉模拟评分法(VAS)评分以及髋关节Harris评分上,要优于DHS组,有统计学意义(P<0.05)。结论:应用PFNA微创治疗82例高龄日常行动能力低股骨粗隆间骨折,能明显缓解患者术后疼痛及后位的发生,并且PFNA微创螺钉具有创伤小、设计合理、允许病人早期活动,是骨质疏松股骨粗隆间骨折内固定物老年患者理想的内固定物。在日常行动能力患者中疗效明显,值得临床推广。

Objective:To discuss a minimally invasive treatment of intertrochanteric fractures using PFNA in low daily patient. Method:82 cases suffering from intertrochanteric fractures treated in our hospital during May 2009-October 2013(man 33 cases , women 49 cases) were selected. the age of patient between 73-92 years, mean 81 years,We recorded the operative time, blood loss, complications of each patient and evaluate the efficacy of treatment between DHS group and PFNA group .Result:the operative time of the patients was 45-110min, mean 55 ± 16min;blood loss was 80 to 210 ml,;hospital days was 7-16 days, mean 8 days;the load time was 4-8 weeks,;the healing of fracture was 8 to 16 weeks;the whole patients actieve the healing of fracture and had no internal fixation of hip cut, screw nails piercing or other complications,41 cases treated by PFNA minimally invasive have statistically significant (P <0.05) in postoperative bleeding, ambulation time, pain visual analog scale (VAS) score and Harris hip score compared with DHS group Conclusion:PFNA is less invasion, reasonable design and allowing patients functional exercise. PFNA treatment of intertrochanteric fractures is the ideal surgical method during osteoporosis elderly suffering from unstable intertrochanteric fracture, especially in low daily patient.
