目的：探讨在线血液透析滤过（ on-line hemodiafiltration ，OLHDF）对维持性血液透析患者微炎症和营养状况的影响。方法选择维持性血液透析的终末期肾病患者40例，随机分为2组：OLHDF组和HD组，每组20例，分别于治疗前和治疗3个月后采血检测血清白蛋白（albumin，Alb）、C-反应蛋白（c-reactiveprotein，CRP）、白介素6（inter-leukin-6，IL-6）水平。结果 HD组与OLHDF组患者治疗前血CRP、IL-6和Alb差异均无统计学意义（ P ＞0．05）。治疗后，HD组患者CRP和IL-6水平较治疗前无明显改变（ P ＞0．05）；OLHDF组治疗后血CRP和IL-6水平较治疗前均明显下降（ P ＜0．05）。HD 组和OLHDF组治疗后较治疗前Alb均升高（ P ＜0．05）。 OLHDF组治疗后Alb升高幅度较HD组明显（ P ＜0．05），CRP和IL-6水平下降幅度较HD组大（ P ＜0．05）。结论 OLHDF治疗维持性血液透析患者可有效清除中大分子炎症因子，减轻微炎症状态，改善MHD患者的营养状况。
목적：탐토재선혈액투석려과（ on-line hemodiafiltration ，OLHDF）대유지성혈액투석환자미염증화영양상황적영향。방법선택유지성혈액투석적종말기신병환자40례，수궤분위2조：OLHDF조화HD조，매조20례，분별우치료전화치료3개월후채혈검측혈청백단백（albumin，Alb）、C-반응단백（c-reactiveprotein，CRP）、백개소6（inter-leukin-6，IL-6）수평。결과 HD조여OLHDF조환자치료전혈CRP、IL-6화Alb차이균무통계학의의（ P ＞0．05）。치료후，HD조환자CRP화IL-6수평교치료전무명현개변（ P ＞0．05）；OLHDF조치료후혈CRP화IL-6수평교치료전균명현하강（ P ＜0．05）。HD 조화OLHDF조치료후교치료전Alb균승고（ P ＜0．05）。 OLHDF조치료후Alb승고폭도교HD조명현（ P ＜0．05），CRP화IL-6수평하강폭도교HD조대（ P ＜0．05）。결론 OLHDF치료유지성혈액투석환자가유효청제중대분자염증인자，감경미염증상태，개선MHD환자적영양상황。
Objective To investigate the effect of online hemodiafiltration on micro -inflammation and nutritional statusin maintenance hemodialysis patients .Method s Forty maintenance hemodialysis patients with end-stage renal disease (ESRD) were randomly divided into two groups:OLHDF group and HD group,with 20 patients in each group.The serum levels of albumin(Alb),C-reactive protein(CRP) and interleukin-6(IL-6) were detected before treatment and 3 months after treatment.Results Before treatment there were no significant differences in the serum levels of Alb ,CRP and IL-6 between HD group and OLHDF group( P >0.05).After treatment there were no significant differences in the serum levels of CRP and IL-6in HD group,as compared with those before treatment ( P >0.05 ),however,the serum levels of CRP and IL-6 were significantly decreased in OLHDF group,as compared with those before treatment ( P <0.05).After treatment the levels of Alb in HD and OLHDF group were obviously increased ,as compared with those before treatment( P <0.05),moreover,the increase amplitude of Alb in OLHDF group was more obvious than that in HD group ( P <0.05),however,the decrease amplitude of CRP and IL-6 in OLHDF group was more obvious than that in HD group ( P <0.05).Conclusion OLHDF can effectively clear the median-macromolecules inflammatory factors in maintenance hemodialysis patients ,relieve micro-inflamma-tory state and improve the nutritional status of MHD patients .
