目的:探讨气道感觉神经末梢(主要是C纤维末梢)释放的P物质(SP)在感染后咳嗽(CPI)发病机制中的作用与地位,并阐述其与肺功能之间的关系。方法:选取2012年5月-2013年12月深圳市沙河医院内科门诊及住院患者37例作为病例组,另外选取35例健康成人志愿者作为正常对照组,观察两组0、28 d痰上清液中SP水平,测试相应时间点肺功能的情况。结果:病例组内28 d痰上清液中SP较0 d明显升高(P<0.05),且组内28 d对应时间点的FEV1及FEV1/FVC较0 d均明显下降(P<0.05)。病例组0、28 d痰上清液中SP较正常对照组均明显升高(P<0.01),同样,病例组28 d对应时间点的FEV1及FEV1/FVC较正常对照组的28 d均明显下降(P<0.01)。由此可见,肺功能中FEV1及FEV1/FVC指标与SP呈反相关(P<0.05)。结论:气道敏感性增高并伴随以痰上清液中SP增高为特点的气道神经源性炎症是CPI的重要特征,可能在CPI咳嗽发病机制中起到重要作用。

목적:탐토기도감각신경말소(주요시C섬유말소)석방적P물질(SP)재감염후해수(CPI)발병궤제중적작용여지위,병천술기여폐공능지간적관계。방법:선취2012년5월-2013년12월심수시사하의원내과문진급주원환자37례작위병례조,령외선취35례건강성인지원자작위정상대조조,관찰량조0、28 d담상청액중SP수평,측시상응시간점폐공능적정황。결과:병례조내28 d담상청액중SP교0 d명현승고(P<0.05),차조내28 d대응시간점적FEV1급FEV1/FVC교0 d균명현하강(P<0.05)。병례조0、28 d담상청액중SP교정상대조조균명현승고(P<0.01),동양,병례조28 d대응시간점적FEV1급FEV1/FVC교정상대조조적28 d균명현하강(P<0.01)。유차가견,폐공능중FEV1급FEV1/FVC지표여SP정반상관(P<0.05)。결론:기도민감성증고병반수이담상청액중SP증고위특점적기도신경원성염증시CPI적중요특정,가능재CPI해수발병궤제중기도중요작용。
Objective:To investigate the effect of the level of SP(released from airway sensory nerve endings, mainly C nerve fiber) in CPI pathogenesis and elaborate its relation with pulmonary function.Method:37 CPI patients outpatient and inpatient in Shenzhen Shahe hospital were selected as the case group,and 35 healthy adult volunteers were selected as the normal control group,observed their SP levels of in sputum supernatant on 0 d and 28 d and tested the pulmonary function at the same time.Result:In the cases group,the SP level of in sputum supernatant at 28 d was significantly higher than that at 0 d(P<0.05),the corresponding time point of FEV1,FEV1/FVC at 28 d were significantly lower than those at 0 d(P<0.05).The SP levels of in sputum supernatant at 0,28 d were significantly higher than those in the normal control group(P<0.01),also the corresponding time point of FEV1,FEV1/FVC at 28 d in the case group were were significantly lower than those in the normal control group at 0 d(P<0.01).Thus FEV1 and FEV1/FVC indexes in Pulmonary Function Test were negatively related to SP in sputum supernatant.Conclusion:Airway neurogenic inflammation signified with increasing airway sensitivity and SP level is an important character of CPI and may play a key role in its pathogenesis.
