背景:股骨头坏死保头治疗的关键在于重建并改善股骨头内的血运,保留股骨头的完整性,恢复股骨头的力学环境和血供,中药具有整体优势。高位头颈开窗支撑植骨能够恢复股骨头内的力学环境。
<br> 目的:观察活血补肾中药联合高位头颈开窗植骨支撑治疗非创伤性股骨头坏死的临床疗效。
<br> 方法:采用前瞻对比试验设计方案,将非创伤性股骨头坏死患者分为两组:治疗组52例60髋口服活血补肾方联合高位头颈开窗植骨支撑治疗,自拟活血补肾方由临沂市人民医院股骨头专科自制;对照组51例60髋单纯采用高位头颈开窗植骨支撑治疗。随访12个月,从末次随访髋关节功能Harris评分及其优良率两方面对疗效进行评价。
<br> 结果与结论:治疗组患者Harris总分、疼痛程度、关节活动范围分值均显著高于对照组(P <0.01);两组关节畸形、功能分值差异无显著性意义(P>0.05);治疗组患者疗效优良率显著高于对照组(88%,74%,χ2=4.54, P <0.05)。提示活血补肾中药联合高位头颈开窗植骨支撑治疗非创伤性股骨头坏死疗效肯定,在缓解或减轻疼痛、改善髋关节活动范围等方面优于单纯高位头颈开窗植骨支撑治疗。

<br> 방법:채용전첨대비시험설계방안,장비창상성고골두배사환자분위량조:치료조52례60관구복활혈보신방연합고위두경개창식골지탱치료,자의활혈보신방유림기시인민의원고골두전과자제;대조조51례60관단순채용고위두경개창식골지탱치료。수방12개월,종말차수방관관절공능Harris평분급기우량솔량방면대료효진행평개。
<br> 결과여결론:치료조환자Harris총분、동통정도、관절활동범위분치균현저고우대조조(P <0.01);량조관절기형、공능분치차이무현저성의의(P>0.05);치료조환자료효우량솔현저고우대조조(88%,74%,χ2=4.54, P <0.05)。제시활혈보신중약연합고위두경개창식골지탱치료비창상성고골두배사료효긍정,재완해혹감경동통、개선관관절활동범위등방면우우단순고위두경개창식골지탱치료。
BACKGROUND:The treatment of osteoporosis of the femoral head relies on the reconstruction and improvement of blood supply in the femoral head, maintenance of femoral head structure, restoration of mechanical environment and blood supply in femoral head. Traditional Chinese medicine treatment is very advantageous. Bone impaction grafting through a femoral neck window at high location can restore the mechanical environment in femoral head.
<br> OBJECTIVE:To investigate clinical efficacy of combined treatment of HUOXUEBUSHEN decoction and bone impaction grafting through a femoral neck window at high location for non-traumatic osteoporosis of the femoral head.
<br> METHODS:This study is a prospective control ed trial. The patients with non-traumatic osteoporosis of the femoral head were divided into two groups:treatment group (n=52, 60 hips) and control group (n=51, 60 hips). The treatment group was given a combined treatment of HUOXUEBUSHEN decoction and bone impaction grafting through a femoral neck window (the decoction was prepared in People’s Hospital of Linyi, China). The control group underwent simple bone impaction grafting through a femoral neck window. Al the involved patients were fol owed up for 12 months, Harris hip score and the excellent-good rate at the last fol ow-up were used to evaluate clinical efficacy.
<br> RESULTS AND CONCLUSION:The Harris score, pain degree, and joint range of motion scores in the treatment group were higher than that in control group (P<0.01), but joint deformities and functional scores showed no significant differences between two groups (P>0.05). The excellent-good rate in the treatment group was significantly higher than that in control group (88%, 75%;χ2=4.91, P<0.05). The combined treatment of HUOXUEBUSHEN decoction and bone impaction grafting through a femoral neck window at high location is effective in the treatment of non-traumatic osteonecrosis of the femoral head, and is better than simple bone impaction grafting at al eviating pain and improving joint range of motion.
