目的:探讨乌司他丁(ulinastatin, UTI)对脂多糖所致内毒素血症大鼠胃肠黏膜循环的影响。方法30只雄性Wistar 大鼠随机分成3组,正常对照组( NC组)、脂多糖( Lipopolysaccharide , LPS)处理组( LPS组)和乌司他丁处理组( UTI组),每组10只。正常对照组只麻醉不处理,脂多糖组和乌司他丁组经腹腔注射脂多糖5 mg/kg,乌司他丁组于脂多糖注射前经尾静脉静注乌司他丁50000 U/kg。分别于处理前、脂多糖注射后3、6和12 h检测胃黏膜pH( pHi)值,同时经股静脉抽血检测肿瘤坏死因子-α( TNF-α)、白细胞介素-6( IL-6)。采用SPSS 13.0软件进行统计学分析,结果以均数±标准差( x珋±s)表示。各时间点组间比较采用单因素方差分析,P<0.05为差异有统计学意义。结果经脂多糖处理后,大鼠3 h后血浆TNF-α和IL-6含量即开始逐渐升高,胃黏膜pH逐渐降低;与脂多糖组比,乌司他丁组在T3、T6和T12时间点TNF-α含量明显降低( T3: P=0.044; T6: P=0.035;T12:P=0.033), IL-6含量明显降低(T3:P=0.041; T6: P=0.022; T12: P=0.024),胃黏膜pH明显升高(T3:P=0.003;T6:P=0.028;T12:P=0.012)。结论乌司他丁可降低内毒素血症大鼠外周血TNF-α和IL-6浓度,增加胃黏膜pH值,改善胃肠黏膜灌注。

목적:탐토오사타정(ulinastatin, UTI)대지다당소치내독소혈증대서위장점막순배적영향。방법30지웅성Wistar 대서수궤분성3조,정상대조조( NC조)、지다당( Lipopolysaccharide , LPS)처리조( LPS조)화오사타정처리조( UTI조),매조10지。정상대조조지마취불처리,지다당조화오사타정조경복강주사지다당5 mg/kg,오사타정조우지다당주사전경미정맥정주오사타정50000 U/kg。분별우처리전、지다당주사후3、6화12 h검측위점막pH( pHi)치,동시경고정맥추혈검측종류배사인자-α( TNF-α)、백세포개소-6( IL-6)。채용SPSS 13.0연건진행통계학분석,결과이균수±표준차( x류±s)표시。각시간점조간비교채용단인소방차분석,P<0.05위차이유통계학의의。결과경지다당처리후,대서3 h후혈장TNF-α화IL-6함량즉개시축점승고,위점막pH축점강저;여지다당조비,오사타정조재T3、T6화T12시간점TNF-α함량명현강저( T3: P=0.044; T6: P=0.035;T12:P=0.033), IL-6함량명현강저(T3:P=0.041; T6: P=0.022; T12: P=0.024),위점막pH명현승고(T3:P=0.003;T6:P=0.028;T12:P=0.012)。결론오사타정가강저내독소혈증대서외주혈TNF-α화IL-6농도,증가위점막pH치,개선위장점막관주。
Objective To explore the effect of ulinastatin on the gastrointestnal circulation in rats with sepsis induced by LPS.Methods Thirty adult Wistar rats were randomly divided into three groups (n=10):normal control group (group NC),LPS group (group LPS) and ulinastatin group (group UTI).Group NC received only anesthesia.The rats in group LPS and group UTI received 5 mg/kg of LPS.5 minutes before the injection of LPS, rats in the group UTI received 50 000U/kg of ulinastatin.The blood of the femoral vein was collected to determine the levels of TNF-αand IL-6, and the pH of gastric mucosa was measured at 0, 3, 6 and 12 hours after processing .The experimental results are expressed as mean ±SD.Statistical analyses were carried out by using SPSS 13.0.One-way ANOVA was used to compare the results of the groups .Differences were considered to be statistically significant when P<0.05.Results After 3 hours of LPS treatment , plasma TNF-αand IL-6 levels of the rats began to rise gradually , the pH of gastric mucosa gradually reduced . Compared with group LPS, the levels of TNF-αin group UTI were significantly reduced ( T3:P=0.044;T6:P=0.035;T12:P=0.033), the levels of IL-6 were significantly reduced(T3:P=0.041;T6:P=0.022;T12:P=0.024), and the pH of gastric mucosa was significantly increased (T3:P=0.003;T6:P=0.028;T12:P=0.012).Conclusion Ulinastatin could improve systemic inflammatory response syndrome and gastrointestinal blood perfusion in rats with sepsis , with reduced TNF-αand IL-6 and increased pH .
