病人就诊中,及时的图像检查对病情确诊至关重要。如何通过合理有效的调度,使有限的图像设备能够满足大部分病人在目标等待时间内完成检查,是医院管理者亟待解决的问题。考虑两类检查项目、三类病人以及目标等待时间为一天的情况,建立了有限时域马尔科夫决策支持模型,探讨病人的最优预约调度策略。在模型中考虑将同一个病人的两类检查安排在同一天。数值试验发现,对于只需一项检查的病人的最优控制策略是随状态变化的开关曲线,即当状态降至该曲线之下(或之左),安排病人检查,否则拒绝病人。最优函数呈现随状态非增变化的特征。最后,敏感度分析考虑了收益及病人到达概率对最优控制策略影响。

Timely imaging examination is critical for the diagnosis of patients.It’s important for the hospital managers to meet the waiting time targets of outpatients by effective and efficient appointment scheduling.A finite horizon Markov Decision Process is proposed to determine the optimal control policy for the appointment of outpatients,by considering two types of examinations and three types of outpatients with waiting time target of one day.The scheduling of two types of examinations for the same patients is to be scheduled to the same day.Numerical results show that the optimal control policy for outpatient with one examination is a switch curve policy that varies with the states,that is,when the states fall below (or left)the curve,the optimal control is to accept the request;otherwise,to reject the request.The optimal function is non-increasing with the increasing of state variables.Finally,the sensitivity analysis explores the impact of revenue and probability of arrivals on the optimal control policy.
