目的:观察早期血浆置换对重症高脂血症性胰腺炎的治疗效果。方法选取临床确诊的36例重症高脂血症性胰腺炎患者,随机分为两组:对照组20例采用内科传统治疗;治疗组16例在内科传统治疗的基础上给予早期单次床旁血浆置换治疗。比较两组血常规、血清淀粉酶、血糖、甘油三酯、C-反应蛋白(CRP)、降钙素原(PCT)、白细胞介素-6(IL-6)、可溶性细胞间黏附分子-1(sICAM 1-)等指标的变化。结果经治疗后两组血常规、血淀粉酶、血糖、甘油三酯、CRP、PCT、IL-6和sICAM-1等指标均有不同程度的改善,其中治疗组改善尤为明显,较治疗前和对照组治疗后差异有统计学意义( P均<0.05)。结论早期行血浆置治疗急性高脂血症性重症胰腺炎能有效减轻炎症反应,阻止病情发展。

목적:관찰조기혈장치환대중증고지혈증성이선염적치료효과。방법선취림상학진적36례중증고지혈증성이선염환자,수궤분위량조:대조조20례채용내과전통치료;치료조16례재내과전통치료적기출상급여조기단차상방혈장치환치료。비교량조혈상규、혈청정분매、혈당、감유삼지、C-반응단백(CRP)、강개소원(PCT)、백세포개소-6(IL-6)、가용성세포간점부분자-1(sICAM 1-)등지표적변화。결과경치료후량조혈상규、혈정분매、혈당、감유삼지、CRP、PCT、IL-6화sICAM-1등지표균유불동정도적개선,기중치료조개선우위명현,교치료전화대조조치료후차이유통계학의의( P균<0.05)。결론조기행혈장치치료급성고지혈증성중증이선염능유효감경염증반응,조지병정발전。
Objective To investigate the effects of earlier bedside plasma exchange on patients with hyperlipidemic pancreatitis .Methods Thirty -six patients with severe hyperlipidemic pancreatitis were randomly divided into observation group and control group .The 16 patients in observation group were treated with conventional therapy plus bedside plasma exchange , and the 20 patients in control group were treated with conventional ones.The changes of white blood cell(WBC),serumamylase(AMY),bloodglucose (GLU), triglyceride(TG),C-reactiveprotein(CRP),procalcition(PCT),interleukin -6(IL-6)and soluble intercellular adhesion molecule( sICAM -1 ) in the two groups were compared .Results After treatment, the laboratory results in the two groups were improved in different degree , but the observation group were improved significantly(p<0.05).Conclusions Earlier bedside plasma exchange for the treatment of patients with severe hyperlipidemic pancreatitis can alleviate inflammation reaction and stop disease progression .
