目的：利用磁共振弥散张量成像（ DTI）技术研究脑出血患者偏瘫肢体功能障碍情况。方法运用DTI技术检测32例脑出血患者发病3周左右病变侧皮质脊髓束（ CST ）受损情况，分析其与患肢Brunnstrom评分及NIHSS评分之间的关系。结果 DTI所显示的CST受损程度与Brunnstrom评分呈负相关（P＜0．001），与NIHSS评分呈正相关（P＜0．001）。结论 DTI可直观显示与脑出血患者肢体运动功能障碍密切相关的CST受损情况，有助于病情的客观评估。
목적：이용자공진미산장량성상（ DTI）기술연구뇌출혈환자편탄지체공능장애정황。방법운용DTI기술검측32례뇌출혈환자발병3주좌우병변측피질척수속（ CST ）수손정황，분석기여환지Brunnstrom평분급NIHSS평분지간적관계。결과 DTI소현시적CST수손정도여Brunnstrom평분정부상관（P＜0．001），여NIHSS평분정정상관（P＜0．001）。결론 DTI가직관현시여뇌출혈환자지체운동공능장애밀절상관적CST수손정황，유조우병정적객관평고。
Objective To valuate the application of diffusion tensor imaging ( DTI ) in analyzing hemiplegia in patients with hypertensive intracranial hemorrhage .Methods DTI techniques were applied to detect the damage extent of the corticospinal tracts ( CST) in 32 patients after 3 weeks from onset .Furthermore , the relationship among the damage extent of CST , NIHSS scores and Brunnstrom limb scores was analyzed .Results A highly negative correlation was found between the damage extent of CST and Brunnstrom limb scores , while a high positive correlation was also found between the damage extent of CST and NIHSS scores ( P<0 .001 ) .Conclusion DTI could directly demonstrate the damage extent of CST which are closely related to the motor dysfunction in patients with hypertensive intracranial hemorrhage .
