目的:探讨早期应用盐酸氨溴索治疗早产儿呼吸窘迫综合征的临床效果。方法早产儿患儿为2006年8月至2013年7月间住院病例共110例,按随机数字表法分2组,对照组(n=55)按常规治疗,研究组(n=55)在常规治疗的基础上加用大剂量盐酸氨溴索静脉滴注。结果研究组治疗总有效率为90.9%,对照组为72.7%,2组比较差异有统计学意义(P<0.05)。研究组患儿呼吸急促、呼吸困难、紫绀等临床症状缓解时间、给氧时间、住院时间及治疗后氧分压、二氧化碳分压、氧合指数等与对照组比较差异具有统计学意义(P<0.05)。结论早期大剂量应用盐酸氨溴索对早产儿呼吸窘迫综合征具有较好的治疗效果,不仅具有促进肺泡表面活性物质生成,还有恢复气道通畅、保护气道组织、减少用氧时间等综合作用,从而更快更好地改善患儿不良症状,加快病情康复。

목적:탐토조기응용염산안추색치료조산인호흡군박종합정적림상효과。방법조산인환인위2006년8월지2013년7월간주원병례공110례,안수궤수자표법분2조,대조조(n=55)안상규치료,연구조(n=55)재상규치료적기출상가용대제량염산안추색정맥적주。결과연구조치료총유효솔위90.9%,대조조위72.7%,2조비교차이유통계학의의(P<0.05)。연구조환인호흡급촉、호흡곤난、자감등림상증상완해시간、급양시간、주원시간급치료후양분압、이양화탄분압、양합지수등여대조조비교차이구유통계학의의(P<0.05)。결론조기대제량응용염산안추색대조산인호흡군박종합정구유교호적치료효과,불부구유촉진폐포표면활성물질생성,환유회복기도통창、보호기도조직、감소용양시간등종합작용,종이경쾌경호지개선환인불량증상,가쾌병정강복。
Objective To investigate the clinical efficacy of early application of ambroxol hydrochloride in the treatment of respiratory distress syndrome in premature infants.Methods A total of 110 premature infants hospitalized from August 2006 to July 2013 were randomly treated with conventional therapy alone(control group,n=55)or in combination with intravenous drip of large dose ambroxol hydrochloride(study group,n=55).Results The total effective rate in study group was significantly higher than that in control group(90.9% vs 72.7%,P<0.05).There were obvious differences in the time to alleviation of clinical symptoms(polypnea,dyspnea,cyano-sis,etc.),duration of oxygen therapy,length of hospital stay,partial pressure of oxygen,partial pressure of carbon dioxide and oxygenation index between the two groups(P<0.05).Conclusion The early application of ambroxol hydrochloride can promote the production of pulmonary surfac-tant,keep airway unobstructed,protect airway tissue and reduce oxygen administration in the treatment of respiratory distress syndrome in premature infants.Therefore,the treatment with ambroxol hydrochloride is effective for improving adverse symptoms and accelerating recovery in premature infants with respiratory distress syndrome.
