目的:探讨骨挤压器行上颌窦底内提升不植骨同期植入种植体术在口腔种植中的应用及临床效果。方法15例上颌后牙缺失者,种植区剩余牙槽骨高度(residual bone high,RBH)为4~8 mm,用骨挤压器行上颌窦底内提升不植骨同期牙种植术,共植入ITI种植体17颗,术后6个月行种植体上部冠修复。通过两年的临床观察,分析种植体存留率、评价种植体周围软组织健康状况,拍摄X线片评价种植体与周围骨组织结合状况、种植体颈部边缘骨吸收状况和窦底提升后新骨形成状况。结果观察期内17颗种植体存留率为100%,术后6个月X线片检查种植区窦底平均提升高度为3.12 mm(2.03~4.13 mm),种植体骨结合良好,术后每次X线复查种植体颈部周围骨水平吸收<0.2 mm。结论骨挤压器行上颌窦底提升不植骨同期种植术方法可靠,效果肯定,能有效治疗窦底RBH不足的上颌后牙缺失者。

목적:탐토골제압기행상합두저내제승불식골동기식입충식체술재구강충식중적응용급림상효과。방법15례상합후아결실자,충식구잉여아조골고도(residual bone high,RBH)위4~8 mm,용골제압기행상합두저내제승불식골동기아충식술,공식입ITI충식체17과,술후6개월행충식체상부관수복。통과량년적림상관찰,분석충식체존류솔、평개충식체주위연조직건강상황,박섭X선편평개충식체여주위골조직결합상황、충식체경부변연골흡수상황화두저제승후신골형성상황。결과관찰기내17과충식체존류솔위100%,술후6개월X선편검사충식구두저평균제승고도위3.12 mm(2.03~4.13 mm),충식체골결합량호,술후매차X선복사충식체경부주위골수평흡수<0.2 mm。결론골제압기행상합두저제승불식골동기충식술방법가고,효과긍정,능유효치료두저RBH불족적상합후아결실자。
Objective To investigate the effect of osteotome sinus floor elevation with immediate implantation and without bone graft-ing in the implant restoration.Methods 15cases with 4~8mm height of residual bond in the posterior maxilla were placed maxillary sinus lifting techniques and simultaneous implant 17 ITI placements without bone grafting.The implants were restored after six months.All the pa-tients were followed up for 2 years to evaluate the implant survival rate and the peri-dental implant tissue condition.X-ray measurements were performed to evaluate bone remodeling around the implant.Results The cumulated survival rate was 100%.Six months later,it was found that bone height of sinus floor on implants was elevated about 3.12mm,and the alveolar bone was well osteointegrated with the implants.Each X-ray examination of bone around the implant neck level absorption was less than 0.2mm.Conclusion The technique of osteotome sinus floor elevation and immediate dental implantation without bone grafting has satisfactory therapeutic effect.It could be used in the maxillary posterior area implant placement.
